弹性髓内钉结合人工骨治疗单纯性骨囊肿

摘　　要：目的探讨弹性髓内钉结合人工骨植骨治疗单纯性骨囊肿的疗效及特点。方法运用骨囊肿刮除人工骨植骨弹性髓内钉内固定治疗单纯性骨囊肿共19例,其中肱骨近端13例,股骨近端6例。根据Neer标准活跃性8例,非活跃性11例。术后骨囊肿稳定后1年拆除弹性髓内钉。结果所有病例均获9-41月的随访,平均27.3月。参照Capanna骨囊肿愈合结果评定,完全愈合14例(73.7%),部分愈合5例(26.3%)。愈合时间3-7月,平均4.2月。19例除1例出现皮肤激惹,其余未发生感染、人工骨排异反应、骨不愈合等并发症。结论使用弹性髓内钉结合人工骨治疗单纯性骨囊肿不但可以有效地防治病理性骨折,而且愈合快,愈合率高,是一种临床上可行、安全的有效方法。Objective To study the effects and advantages of elastic stable intramedullary nail （ESIN） combined with antificial bone graft for the treatment of simple bone cysts. Methods Ninteen cases of sirnple bone cyst were treated by ESIN combined with antifieial bone graft. Thirteen cysts were located in proximal humerus ,6 in the proximal femur. Eight cysts were active and 11 inactive according to Neer standard. Internal fixtor was removed I year afer bone cyst was stable. Results All the patients were followed up for 9 to 41 months,averaged 27.3 months. Fourteen cysts（73.7% ） healed completely,and 5 cysts（26.3% ） partly healed according to Capanna standard. Bone cysts union ranged from 3-7 months, averag of 4.2 months without complications as infection, rejection reaction and nonunion except one patient with nfild irritation of skin caused by the end of the nail. Conclusions ESIN combined with bone graft for treatment of simple bone cysts not only has satisfactory healing rate but also can accelerate the healing of cysts, which provides prevention and treatment of pathological fractures. And it is considered to be a safe,viable and effective method.
