原发性干燥综合征伴间质性肺病患者血清基质金属蛋白酶-7水平及其意义的研究

摘　　要：目的探讨基质金属蛋白酶（MMP）-7在原发性干燥综合征（pSS）伴间质性肺病（ILD）患者血清水平及其临床意义。方法筛选50例pSS患者，其中27例患者无系统受累证据（单纯DSS组）；23例患者中系统受累仅表现为肺间质改变（pSS＋ILD组）。15名健康献血者为对照。酶联免疫吸附试验（ELISA）法检测血清中MMP-7的水平，评价MMP-7与临床指标的相关性。采用t检验和单因素方差分析，Pearson相关分析进行统计学处理。结果3组间MMP-7水平差异有统计学意义（F=9.599，P〈0.01）。单纯pSS组及pSS＋ILD组患者的血清MMP-7水平分别为（3.58±1.61）ng]ml、（4.63±1.67）ng／ml，均显著高于健i康对照组的（2.34±0.21）ng／ml（P〈0.01）。进一步的组问比较显示，pSS＋ILD组血清MMP-7水平显著高于单纯pSS组（P〈0.05）。相关分析显示单纯pSS组及DSS＋ILD组血清MMP-7与c反应蛋白水平呈正相关（r=0.42，0.58；P均〈0.05），而与用力肺活量、一口气弥散量呈负相关（r=0.52，-0.46；P〈O.05）。结论MMP-7在pSS及其所合并的ILD中可能具有重要作用。Objective To investigate the serum level and the clinical significance of matrix metalloproteinases （MMP）-7 in patients with primary Sj6gren＇s syndrome （pSS） complicated with interstitial lung disease （ILD）. Methods Fifty untreated patients with pSS were enrolled in the study. Of them, 27 patients were only lacriminal and salivary gland involvement （simple pSS group）. The other 23 patients were lacriminal and salivary gland involvement as well as being concomitant with ILD （pSS＋ILD group）. Fifteen healthy blood donors were used as controls. The serum MMP-7 levels were measured by ELISA. Indepent t-test and one-way ANOVA were used for date analysis, and Pearson＇s test was adopted for correlation analysis. Results The mean serum level of MMP-7 in patients with ILl） was significantly higher than that in the patients with simple pSS [ （3.58±1.61） ng/ml vs （4.63 ±1.67） ng/ml, P〈0.05 ]. The levels of serum MMP-7 in the two patient groups was both increased significantly when compared with that in healthy controls （2.34±O.21） ng/ml, （both P〈0.01）. Correlation analysis showed that the serum MMP-7 level were correl- ated with the serum C-reactive protein levels in both patient groups （r=0.42, 0.58; P〈0.05）, as well as negatively correlated with forced vital capacity and breath dispersion quantity （r=0.52, -0.46; all P〈0.05）.
