“靳三针”疗法治疗缺血性中风后痉挛性偏瘫随机对照研究

摘　　要：目的观察"靳三针"疗法、康复疗法、"靳三针"疗法结合康复疗法对缺血性中风后痉挛性偏瘫患者的治疗效果,研究和评价"靳三针"疗法对缺血性中风后痉挛性偏瘫的治疗作用,为进一步探讨治疗中风后痉挛性偏瘫的较优方法提供依据。方法将141例符合诊断标准、纳入标准和排除标准,发病在2星期到6个月的缺血性中风后痉挛性偏瘫患者,随机分为康复组、靳三针组和综合组(靳三针疗法结合康复疗法组)。观察患者治疗前后日常生活能力评分和四肢运动功能评分的改善,并比较3组疗效。结果 3组患者治疗后日常生活能力和四肢运动功能评分值均有所提高(P<0.001)。以综合组评分值改善最大(P<0.05),康复组和靳三针组改善值相当(P>0.05)。结论在改善缺血性中风后痉挛性偏瘫患者日常生活能力和四肢运动功能方面,康复疗法和靳三针疗法的疗效相当;靳三针疗法结合康复疗法的疗效优于单纯康复疗法和靳三针疗法。靳三针疗法与康复疗法结合运用是促进缺血性中风偏瘫患者日常生活能力和运动功能改善和恢复的较优方法。Objective To observe the therapeutic effect of Jin＇s three needle therapy, rehabilitation training, and Jin＇s three needle therapy plus rehabilitation training on spastic hemiplegia after ischemic stroke, and to evaluate the treatment effect of Jin＇s three needle therapy for spastic hemiplegia after ischemic stroke, for providing evidence for the further exploration of a superior method in treating post-stroke spastic hemiplegia. Method Totally 141 patients with spastic hemiplegia due to ischemic stroke, who were eligible according to the diagnosis criteria, inclusion criteria, and exclusion criteria and with a disease duration ranging from 2 weeks to 6 months, were randomized into a rehabilitation group, Jin＇s three needle group, and a combined group （Jin＇s three needle plus rehabilitation）. Activity of Daily Living （ADL） and Fugl-Meyer （FMA） were assessed before and after treatment, and the therapeutic effects were compared among the three groups. Result After treatment, three groups all had significant improvements in ADL and FMA scores （P〈0.001）. The combined group achieved the most significant improvements （P〈0.05）, while the rehabilitation group and Jin＇s three needle group had equivalent improvements （P〉0.05）. Conclusion Rehabilitation and Jin＇s three needle therapy have equivalent effects in ameliorating ADL and FMA in patients with post-stroke spastic hemiplegia; combined use of Jin＇s three needle therapy and rehabilitation training has better therapeutic effect than using rehabilitation or Jin＇s three needle therapy alone. Combined use of Jin＇s three needle therapy and rehabilitation training is a comparatively superior method in improving and restoring ADL and motor function of hemiplegia patients after ischemic stroke.
