摘 要:目的观察针刺拮抗肌配合康复治疗缺血性脑卒中后下肢肌张力增高的临床疗效。方法将60例患者随机分为治疗组和对照组,每组30例。治疗组给予针刺拮抗肌加康复训练治疗,对照组给予传统针刺取穴加康复训练。治疗4个疗程后比较两组患者Fugl-Meyer下肢运动功能评分(Fugl-Meyer Assessment Scale,FMA)、日常生活活动能力(Barthel指数)评定、肌张力MAS评定及临床疗效。结果两组患者治疗后FMA评分、Barthel指数及肌张力MAS评定分级均明显改善(P<0.05,P<0.01)。治疗组治疗后FMA评分、Barthel指数及肌张力MAS评定分级与对照组比较差异有统计学意义(P<0.05)。治疗组总有效率为90.0%,对照组总有效率为66.7%,两组比较差异有统计学意义(P<0.05)。结论针刺拮抗肌配合康复治疗缺血性脑卒中后下肢肌张力增高的疗效优于传统针刺配合康复训练。Objective To observe the therapeutic effect of needling antagonistic muscle plus rehabilitation in treating lower-limb hypertonia after ischemic stroke. Method Sixty patients were randomized into a treatment group and a control group, 30 each. The treatment group was intervened by needling antagonistic muscle plus rehabilitation training, and the control group was by acupuncture with conventional acupoint selection plus rehabilitation training. After 4 treatment courses, Fugl-Meyer Assessment Scale (FMA), activity of daily living, and Modified Ashworth Scale (MAS) were evaluated and therapeutic effects were compared between the two groups. Result After treatment, the two groups both had significant improvements in FMA, Barthel Index (BI), and MAS (P〈0.05,P〈0.01). There were marked differences between the two groups in comparing FMA, BI, and MAS scores after treatment (P〈0.05). The total effective rate was 90.0% in the treatment group versus 66.7% in the control group, and the difference was statistically significant (P〈0.05). Conclusion Needling antagonistic muscle plus rehabilitation has better therapeutic effect than conventional acupuncture plus rehabilitation in treating lower-limb hypertonia after ischemic stroke.
