摘　　要：目的:探讨院外延续护理对全膝关节置换术(TKA)患者康复效果的影响。方法:将83例TKA患者按入院时间分成观察组42例和对照组41例,两组患者住院期间均接受常规治疗、护理,护士按健康教育路径表落实系统健康教育,包括疾病的相关知识、饮食、康复锻炼、并发症预防等,出院当天由责任护士给予出院健康宣教。观察组在此基础上实施院外延续护理,通过电话随访的方式对患者进行疾病知识教育、锻炼指导、遵医行为监督等,并定期进行门诊随访。结果:观察组患者出院后3个月、6个月、12个月日常生活活动能力、康复训练依从性明显优于对照组(P<0.05)。结论:对TKA患者实施院外延续护理能提高其康复训练依从性,促进肢体功能恢复,提高生活质量,同时能够深化优质护理服务的内涵,提升护士的综合能力。Objective ： To explore the impact of the extended nursing care outside hospital on the rehabilitation effect of the patients after total knee arthroplasty. Methods ：83 patients with total knee arthroplasty were divided into the observation group （ n = 42）and the control group （n =41 ） according to the admission time. The patients in both groups received routine treatment and nursing care during hospitaliza- tion and nurses provided systematic health education for the patients according to health education path ,including instruction of disease - related knowledge, diet, exercise and prevention of complications. The primary nurse gave patients the certain health education on the right day of discharge. The extended nursing care outside hospital was implemented in the observation group, including instruction of disease - related knowledge ,exercise guidance, supervision of compliance behavior of patients through telephone visit and regular follow - up. Re- sults：The ability of ADL and the patients＇compliance of rehabilitation training were better in the observation group than the control group respectively in 3,6 andl2 months after discharge { P 〈 0.05）. Conclusion ：The extended nursing care outside hospital can improve the pa- tients＂ compliance of rehabilitation training, promote the recovery of limb function and improve the quality of life of the patients after total knee arthroplasty ,it can also deepen the connotation of high quality nursing service and improve the comprehensive ability of nurses.
