特发性室性心律失常患者的临床特征和射频消融手术结果

摘　　要：目的对特发性室性心律失常患者进行系统的回顾性分析,探讨不同起源部位的患者年龄、性别以及手术成功率方面的差异。方法纳入270例行导管射频消融的患者。对每位患者的性别、年龄、主要症状、病史和既往治疗情况等资料进行分析,比较不同起源特发性室性心律失常患者的临床特征和射频消融手术结果。结果 270例患者中男性153例,女性117例,年龄10～82(41.6±16.0)岁。流出道起源的患者中的女性比例较其他部位起源者高(均为P<0.05),左心室间隔部的患者出现症状的年龄为(28.8±11.9)岁,流出道、流入道、其他部位的患者出现症状的年龄分别为(44.8±14.9)岁、(40.2±23.1)岁和(41.5±20.7)岁,左心室间隔部的室性心律失常的患者发病更年轻(均为P<0.05)。女性患者发病年龄集中在31～60岁之间(70.0%),心律失常常起源于右心室流出道(80.3%)。237例患者射频消融手术取得成功,成功率87.8%。结论起源于左、右流出道的患者之间年龄、性别比例差异有统计学意义。在不同年龄阶段的特发性室性心律失常的起源部位构成比不一样。Objective The aim of this study was to compare the clinical features and results of ablation in patients with different original idiopathic ventricular arrhythmias （IVA）. Methods We studied 270 patients [ 153 males and 117 females; mean age （41.6 ± 16. 0） years] with IVA admitted to PLA General Hospital in the past 13 years. All the patients underwent catheter mapping and radiofrequency catheter ablation. The gender, age, symptoms and history of the patients were retrospectively analyzed and clinical features and resuhs of ablation were compared among the different original IVA patients. Results Patients with left ventricular outflow tract （LVOT） original IV A were generally older[ 18-82 （51.4 ± 17.1 ） years] than those with LV-infero-septnm original IVA [ 10-56（28.8 ± 11.9）years, P 〈0. 05]. The onset of the symptom in female patients concentrated in 31 to 60 years old （70. 0% ）, and often originated from the right ventricular outflow tract（ 80. 3% ）. Ablation was successful in 237 patients （87.8%）. Conclusions Distinct gender and age differences are found between ROVT and LOVT original IVA patients. There is a difference in the origins according to the age of the patients.
