冠状位螺旋CT扫描在霉菌性鼻窦炎诊断中的价值

摘　　要：目的:探讨鼻窦霉菌感染的CT特征及临床表现。材料与方法:回顾性分析在我院经手术病理证实的资料完整的14例霉菌性鼻窦炎的冠状位及轴位重建图像表现。结果:受累鼻窦腔内可见软组织肿块充填,病灶内密度不均,部分病灶内可见钙化,病灶内见气泡影,受累鼻窦骨质可见硬化、吸收及破坏。部分病例合并窦口鼻道复合体变窄、阻塞,并见多种变异。结论:鼻窦霉菌感染的主要特征是窦腔内软组织肿块,病灶内散在钙化及气泡影,窦壁骨质增厚或吸收。冠状位CT扫描能较好地显示鼻腔鼻窦的解剖结构和病变范围,基于冠状位螺旋CT而做的轴位重建能更好地弥补冠状位扫描的不足,两者结合对功能性内镜手术有明显的指导意义。Purpose： To explore paranasal sinus mucormycosis infection and clinical manifestation,characteristics of CT.Materials and Methods： For analyzing retrospectively in our hospital confirmed by operation and pathology data of 14 patients with fungal sinusitis coronal CT and axial reconstruction image.Results： The involved sinus cavity is filled with soft tissue tumor,intralesional uneven density,part of intralesional calcification within the lesion,see bubbles containing the artifact,involved sinus bone sclerosis,visible absorption and damage.In some cases complicated with ostiomeatal complex obstruction,and see many variation Conclusion： Paranasal sinus mucormycosis infection is the main feature of the sinus cavity soft tissue mass,intralesional scattered calcification and bubble film,sinus wall bone thickening or absorption.Coronal CT scan can well show the nasal cavity and paranasal sinus anatomy and extent of the lesion,based on the coronal and axial spiral CT reconstruction can better compensate for coronal scan is insufficient,the combination of functional endoscopic operation has obvious guiding significance.
