乙型肝炎血清标志物HBeAg与HBeAb同时阳性模式的相关研究

摘　　要：目的分析乙型肝炎病毒(HBV)血清标志物HBeAg与HBeAb同时阳性患者的病毒复制、肝功能状况,并探讨该类模式的产生原因及其意义。方法应用微粒子化学发光法检测HBV血清标志物,从中筛选出HBeAg与HBeAb同时阳性的标本,检测该类标本HBV-DNA定量值和肝功能结果及肿瘤标志物AFP值。结果检测17 895例患者HBV血清标志物,HBeAg与HBeAb同时阳性73例,阳性率7.4%;73例HBeAg和HBeAb阳性患者中,HBV-DNA>l×E+004拷贝/ml者占55.0%;肝功能指标的异常率为58.3%,肿瘤标志物AFP异常率21.9%。结论乙型肝炎患者出现HBeAg与HBeAb同时阳性,提示乙型肝炎病毒复制处于低水平状态,体内病毒较少,传染性弱,但是也容易造成肝功能的慢性损伤,应当引起实验室和临床的高度重视。OBJECTIVE To analyze the viral replication and liver function of the patients with the simultaneously positive hepatitis B virus （HBV） serum markers HBeAg and HBeAb so as to explore the causes of such patterns and its clinical significance. METHODS The chemiluminescent method was employed to detect the serum markers of HBV, the samples with simultaneously positive hepatitis B serum markers HBeAg and HBeAb were screened out, and the HBVDNA quantitative values of such samples were detected , and the liver function and tumor markers AFP value were observed. RESULTS Of totally 17 895 cases of patients who were tested for the serum markers of HBV, HBeAg and HBeAb were tested simultaneously positive in 73 cases with the the positive rate of 7.4%. In 73 cases of patients with simultaneously positive HBeAg and HBeAb , the patients with the HBV DNA more than 1× E＋004 copies/ml accounted for 55.0% ; the patients with the abnormal liver function accounted for 58.3%, and the patients with the abnormal tumor marker AFP accounted for 21. 9%. CONCLUSION The simultaneously positive HBeAg and HBeAb in the patients with hepatitis B indicates that the viral replication is at a low level, with less virus in the body, less contagious, but it may cause the chronic injury of the liver function, which should be attached great importance.
