老年脑卒中合并院内获得性肺炎的疗效分析

摘　　要：目的分析多学科整合治疗老年脑卒中合并院内获得性肺炎(HAP)的疗效。方法观察组为2010年1月-2012年8月卒中单元收治的85例脑卒中合并HAP的老年患者,进行多学科整合治疗;对照组为同期神经内科收治的80例脑卒中合并HAP老年患者,进行单纯药物治疗,对两组的临床资料进行回顾性分析。结果治疗后观察组卒中量表(NIHSS)和Barthel Index(BI)评分分别为(8.99±3.25)分和(18.31±2.24)分较对照组疗效明显,脑卒中合并HAP临床特征以中低发热为主,咳嗽、咳痰、呼吸困难、食欲不振、淡漠无力等为主要临床表现,病原学检查以革兰阳性菌多见占65.45%,其次为革兰阴性菌占27.88%;观察组与对照组HAP主要症状消失时间、实验室检查正常时间、胸片正常时间、住院天数,差异有统计学意义(P<0.05)。结论多学科整合治疗老年脑卒中合并HAP减少了并发症,缩短住院时间,取得了较明显的治疗效果。OBJECTIVE To analyze the efficacy of integrated multidisciplinary in treatment of cerebral stroke complicated with hospital-acquired pneumonia （HAP） in the elderly patients. METHODS A retrospective study was carried out to investigate the clinical data of 85 cases of elderly stroke patients complicated with HAP in stroke unit （the observation group） and the clinical data of 80 cases of elderly stroke patients complicated with HAP in department of neurology （ the control group）. RESULTS The NIHSS score and Barthel Index （BI） score of the observation group were respectively （24. 01±3.57）and （26.87±3.55）, more significant than the control group. The main clinical manifestation of elderly stroke patients complicated with HAP was low fever, and the cough, expectoration, dyspnea, and anorexia were the main clinical symptoms. The etiological examination showed that the gram-positive bacteria accounted for 65.45 %, the gram-negative bacteria 27.88%. The difference in the time of disappearance of symptom, laboratory examination, normal chest radiograph time, or length of hospital stay between the observation group and the control group was statistically significant （P〈0.05）. CONCLUSION The integrated multidisciplinary can reduce the complications and shorten the length of hospital stay in the treatment of elderly cerebral stroke complicated with HAP, achieving significant therapeutic effect.
