摘 要:目的探讨蒸汽灭菌后器械包包外化学指示物变色不合格发生的根本原因及预防措施,以提高灭菌质量。方法回顾性调查消毒供应中心2011年6-8月经过蒸汽灭菌处理的包外化学指示剂变色不合格的原因并进行分析;2011年9月开始针对其原因采取整改措施,并将2011年10月-2012年3月的统计资料与整改前进行比较。结果整改前的89 862件灭菌包中有63件出现包外化学指示物变色不合格,不合格率为0.702‰,去除原因后处理的155 723件灭菌包中仅有3件出现包外化学不合格,不合格率为0.019‰,差异有统计学意义(χ2=98.59,P<0.01)。结论运用原因分析方法解决问题能有效减少不合格灭菌产品,避免不必要的重复劳动,降低灭菌费用。OBJECTIVE To investigate the root causes and preventive measures of wrapped instrument sets with unqualified external CI (chemical indicator) color-changing after steam processing so as to improve the quality of sterilization. METHODS We retrospectively surveyed the eases of wrapped instrument sets with unqualified external C1 color-changing from Jun to Aug 2011. The root causes were analyzed, and countermeasures were carried out since Sep 2011, then the statistics from Oct 2011 to Mar 2012 were compared with the data before the rectification. RESULTS Before the rectification, totally 63 of 89862 sterilized packs appeared to be unqualified in external color-changing, the occurrence rate was 0. 702%. After the root causes were eradicated, only 3 of 155723 sterilized packs appeared to be unqualified, the occurrence rate was 0. 019 %. the difference was statistically signif- [cant(z2= 98. 59, P〈0. 01). CONCLUSION The application of root causes analysis can reduce the unqualified sterile products effectively and avoid the unnecessary repetitive work as well as cut the costs of sterilization.
