摘 要:目的探讨代谢综合征(MS)患者血清Apelin水平与脂代谢的关系。方法将2009年3月至2009年12月广西医科大学第一附属医院收治的132例就诊患者依据75 g口服葡萄糖耐量试验(OGTT)分为MS患者(69例)和糖调节正常者(63例);前者又分为确诊2型糖尿病(MSⅠ组,35例)和糖调节受损(MSⅡ组,34例),后者按体质量指数(BMI)分为超重/肥胖组(33例)与正常对照组(30例)。采用酶联免疫吸附法测定四组空腹血清Apelin,同时测定总胆固醇(TC)、三酰甘油(TG)、高密度脂蛋白胆固醇(HDL-C)、低密度脂蛋白胆固醇(LDL-C)。结果 MS组空腹血清Apelin水平、ln(HOMA-IR)、TC、TG、LDL-C均显著高于正常糖调节组(P<0.05)。相关分析结果显示,空腹血清Apelin水平与TC、TG、LDL-C、ln(HOMA-IR)均呈正相关(r=0.351,0.235,0.194,0.600,均P<0.05)。多元逐步回归分析显示,ln(HOMA-IR)、BMI和TC是Apelin独立相关因素。结论 MS患者脂代谢异常与血清Apelin水平密切相关,可能与其升高所导致的胰岛素抵抗加重有关。Objective To explore the relationship between the serum apelin and the lipid metabolism in patients with metabolic syndrome( MS). Methods One hundred and thirty-two cases of patient admitted to the First Affiliated Hospital of Guangxi Medical University from March 2009 to December 2009 were divided into MS patients(69 cases)and normal glucose regulation (63)based on a 75 g oral glucose tolerance test (OGTY) ;the former was divided into diagnosed with type 2 diabetes ( MS I group, n = 35 ) and impaired glucose regulation ( MS II group, n = 34 ) , the latter was divided into the overweight/obese group (33 cases ) and control group(30 cases)by body mass index(BMI). Fasting serum apelin of the four groups was tested by enzyme-linked immunosorbent assay, and the simultaneous total cholesterol( TC ), triglyceride (TG), high density lipoprotein cholesterol (HDL-C), and low-density lipoprotein cholesterol(LDL-C) were tested as well. Results The level of serum apelin, In (HOMA-IR) ,TC,TG and LDL-C were higher obviously in MS group (P 〈 0.05 ). Correlation analysis showed that the serum apelin was positively correlated with TC, LDL-C, FINS,In(HOMA-IR) ( r =0. 351,0. 235,0. 194,0. 600, all P 〈 0.05 ). In a muhiple step-wise regression analysis,In (HOMA-IR), BMI and TC were independent factors related to apelin. Conclusion The increased serum apelin is closely related to metabolic abnormalities of lipid of MS patients, which may be related to the insulin resistance caused by the increased apelin.
