新疆生产建设兵团城市和团场门诊与住院服务公平性比较

摘　　要：目的探讨2010年新疆生产建设兵团城市和团场居民门诊和住院利用在不同经济收入人群间分布的公平性。方法按等值人均消费性支出划分不同经济水平人群;计算不同经济水平居民门诊和住院实际利用和标化后利用量的构成及集中指数。结果五等分组人群由贫到富,实际就诊次数的分布城市分别为14.5%、15.8%、19.2%、32.5%和17.9%,团场分别为12.6%、22.2%、17.5%、24.9%和22.7%,趋势不明显,计算其集中指数分别为0.12和0.10;城市和团场住院利用的分布均是随着由贫到富,利用逐渐增加,城市住院次数和住院天数由14%左右增加到24%左右,团场从12%左右增加到30%以上,城市住院利用向富人倾斜程度较团场轻,城市和团场住院次数的集中指数分别为0.13和0.20,住院天数的集中指数分别为0.13和0.17;标化后门诊和住院利用的公平性有不同程度改善。结论城市和团场居民门诊利用的公平性均较好,城市居民住院利用的公平性好于团场。OBJECTIVE To analyze the distribution of outpatient and inpatient utilization among different economic groups of urban and rural residents in Xinjiang Production and Construction Corps in 2010 and evaluate the fairness. METHODS Cal- culated the actual utilization and standardized utilization among different economic groups and concentration index （CI）. RE- SULTS The proportions of actual outpatient utilization of urban residents by each quintile were 14.5%, 15.8%, 19.2%, 32.5% and 17.9% respectively, the proportions of actual outpatient utilization of rural residents were 12.6%, 22.2%, 17.5%, 24.9% and 22.7%, the CI were 0.12 and 0.10 respectively. The proportions of actual inpatient times and days of urban resi- dents increased from 14% to about 24% while the proportions of actual inpatient times and days of rural residents increased from about 12% to over 30%, the CI of actual inpatient times of urban and rural residents were 0.13 and 0.20 respectively and the inpatient days were 0.13 and 0.17. The fairness of standardized outpatient and inpatient utilization had improved. CONCLUSION The fairness of outpatient utilization of urban and rural residents is at a good level, the fairness of inpatient utilization of urban residents is better than that of rural residents.
