摘 要:目的分析某院2006~2010年鲍曼不动杆菌耐药性的变迁与抗菌药物用量之间的关系。方法采用回顾性调查方法,收集该院2006年1月~2010年12月住院患者鲍曼不动杆菌药敏结果及8种抗菌药物使用情况。应用SPSS13.0统计软件对结果进行数据分析。结果鲍曼不动杆菌对哌拉西林/他唑巴坦和亚胺培南耐药率﹤20%。头孢哌酮/舒巴坦5年均保持高度敏感性,耐药率﹤10%,可作为治疗鲍曼不动杆菌的首选。对头孢曲松和左氧氟沙星的耐药率从2006年的64.95%和38.14%上升到2010年的85.08%和52.88%。鲍曼不动杆菌对阿米卡星耐药率与哌拉西林/他唑巴坦用量呈负相关,相关系数为-0.901,差异有统计学意义(P﹤0.05)。对头孢他啶耐药率与头孢哌酮/舒巴坦用量呈正相关,相关系数为0.935,差异有统计学意义(P﹤0.05)。结论鲍曼不动杆菌耐药率逐年增高,耐药水平变化与抗菌药物的使用量有相关性,医院应有规律地轮换、限制抗菌药物,减少多重耐药菌的产生。OBJECTIVE To study the relationship between the drug resistance transition of baumanii and the dosage of an- tibacterials from 2006 to 2010 in our hospital. METHODS The results of drug-sensitive test of baumanii in our hospital from January 2006 to December 2010 and the dosage of eight kinds of antibacterials were analyzed with SPSS 13,0 statistic software by retrospective review. RESULTS The resistance rate of piperacillin and imipenem was less than 20%. Cefoperazone/sulbac- tam had high sensitivity in five years and the resistance rate was less than 10%. They were the first choice to cure baumanii. The resistance rate of ceftriaxone and levofloxaein were increased from 64.95% and 38.14% in 2006 to 85.08% and 52.88% in 2010. Baumannii's resistance to amikaein was negatively correlated to the dosage of piperacillin/tazobactam, the correlation coefficient was -0.901 and the difference had a statistical difference (P 〈 0.05). Its resistance to eeftazidime was positive cor- relation with the dosage of cefoperazone/sulbactam, the correlation coefficient was 0.935 wiht a statistical difference (P 〈 0.05 ). CONCLUSION The drug-resistant of Baumannii is higher year by year and the drug-resistance level was correlated with the dosage of antibacterials. Rational alternate and circumscription of antibiotics should be strengthened to reduce the production of drug-resistant strains.
