地佐辛复合舒芬太尼在脊柱术后自控静脉镇痛中的应用

摘　　要：目的:研究地佐辛复合舒芬太尼在脊柱术后患者自控静脉镇痛(PCIA)中的镇痛效果。方法:将择期行脊柱手术的ASAⅠ～Ⅲ级患者60例,男女不限,随机分为两组,每组各30例。观察组为地佐辛联合舒芬太尼组(DS组),地佐辛0.1mg/kg+舒芬太尼1ug/kg+格拉司琼9mg+0.9%Nacl溶液配成100ml;对照组为舒芬太尼组(S组),舒芬太尼2ug/kg+格拉司琼9mg+0.9%Nacl溶液配成100ml。设定背景流量均为2ml/h,单次自控按压量0.5ml,锁定时间为15min。采用VAS评分法评定术后4、8、12、24、48h疼痛程度,观察不良反应发生情况,记录患者满意度。结果:两组患者均取得满意镇痛效果,DS组患者满意度较好,两组比较差异有统计学意义(P<0.05)。结论:地佐辛复合舒芬太尼用于脊柱手术后的镇痛效果满意且不良反应少。Objective： To evaluate efficacy and safety of dezocine combined with sufentanil in patient-controUed intravenous analgesia（PCIA） for spinal surgery. Methods：Sixty ASA I -111 patients scheduled for spinal surgery under total intravenous anesthesia were randomly divided into observation group and control group with 30 cases each group. Patients in observation group were given dezocine 0. 1 mg/kg＋sufentanil l lμg/kg＋granisetron 9mg, and 100ml NS; while patients in control group were given sufentanil 2ug/kg＋ granisetron 9mg, and 100ml NS. Loading dose of analygesic pump was set as 2 ml, patient-controlled analgesia （PCA）sdoes of 0.5ml and lockout time of 15 min. Visual analogue scale （VAS） score at 4,8,12,24,48h and adverse reaction of two groups were recorded. Results：The analgesic effect was significantly better （P〈0.05）,and the incidence of adverse effect was significantly lower and satisfaction was higher（P〈0.05） in observation group than those in the control group. Conclusion： Dezocine combined with sufentanil is effective on PCIA after spinal surgery, and can obviously relief adverse effect due to sufentanil.
