联合迈之灵治疗慢性前列腺炎伴精索静脉曲张随机平行对照的多中心研究

摘　　要：目的评价联合迈之灵片治疗慢性前列腺炎伴精索静脉曲张患者的疗效。方法2011年8月至2012年3月采用随机、平行对照的研究方法，将330例慢性前列腺炎伴精索静脉曲张患者分为应用α受体阻滞剂及植物药＋迈之灵治疗的观察组259例，单纯应用α受体阻滞剂及植物药的对照组71例。观察用药前和用药后4周、8周慢性前列腺炎症状指数（NIH—CPSI）评分及前列腺按摩液（EPS）中白细胞（WBC）计数情况，对比治疗前后指标进行疗效分析。结果两组患者年龄、教育程度、饮酒、吸烟、病程比较差异均无统计学意义，临床基本情况、精索静脉曲张严重程度和左右侧别比较差异无统计学意义（P〉0．05）。两组患者治疗4周、8周后与治疗前比较，临床症状显著改善，观察组总有效（有效＋显效）率分别为88．7％和95．8％，对照组分别为64．8％和92．8％。治疗4周、8周后的NIH—CPSI评分改善率组间差异有统计学意义（P〈0．05）。观察组有效随访的EPS内WBC达到＋／HP者142例，经过4周和8周治疗后的WBC转阴率分别为26．1％和59．1％；对照组57例，转阴率分别为21．1％和29．8％，两组8周转阴率差异有统计学意义（P〈0．05）。结论联合迈之灵治疗慢性前列腺炎伴精索静脉曲张在改善患者的临床症状，包括疼痛、排尿症状及生活质量方面具有良好的效果，并具有抗炎作用，可以显著降低EPS内的WBC数量。Objective To study the effectiveness of combined Aescuven forte in the treatment of chronic prostatitis with varicocele. Methods In this randomized and parallel comparative study, 330 chronic prostatitis patients with varicocele were divided into observed group with 259 cases and control group with 71 cases. Patients in observed group were treated with α adrenoceptor blocker, phytotherapy and Aes-cuven forte, while patients in and control group were treated with medication only for 8 weeks. Follow up was carried out at 4 and 8 weeks after treatment to evaluate the NIH-CPSI scale and white blood cell （WBC） counts in expressed prostate secretion （EPS）. Results No significant difference was found in age, edu-cation, alcohol consumption, smoking habits, comorbidities and baseline clinical status before treatment be- tween observation group and control group. After 4 week and 8 week treatment, NIH-CPSI score improved significantly in both observation group and control group, P 〈 0.05. The total effective rate （ effective rate ＋ significant effective rate） were 88.7% and 95.8% in observation group and 64.8% and 92.8% in control group, respectively. There were 142 cases with EPS WBC count exceeded ＋/HP in observation group, and 57 cases in control group. After 4 and 8 weeks treatment, negative rate of EPS WBC count were 26.1% and 59.1% in observation group, and 21.1% and 29.8% in control group, respectively. After 8 weeks treat- ment, the negative rate of EPS WBC count in observed group significantly lower than in control group, P 〈 0.05. Conclusion Combined medication treatment with Aescuven forte can achieve favorable effective- ness in symptom improvement and diminishing EPS WBC count in the treatment of chronic prostatitis with varicocele.
