经纤维支气管镜盐酸氨溴索灌洗治疗支原体肺炎并肺不张的疗效及治疗成本分析

摘　　要：目的:探讨经纤维支气管镜盐酸氨溴索灌洗治疗支原体肺炎并肺不张的疗效及经济学效果。方法:采取随机对照方法,将93例支原体肺炎并肺不张患儿分成治疗组和对照组,2组在常规阿奇霉素治疗的情况下,治疗组50例予以生理盐水稀释的盐酸氨溴索灌洗肺不张处;而对照组43例经纤维支气管镜予生理盐水灌洗肺不张处,比较两组患儿发热时间及灌洗前后咳嗽次数,及住院时间,并于2周后复查胸部CT,比较2组疗效,计算治疗成本。结果:2组患儿经过纤维支气管镜灌洗后咳嗽均较灌洗前明显好转(P<0.05),且灌洗后治疗组咳嗽情况较对照组疗效更为明显(18.07±4.78vs28.07±3.39)(P<0.05),治疗费用明显优于对照组(P<0.05)。肺不张有效率治疗组明显高于对照组(P<0.05)。但2组热程无明显差异(P>0.05)。结论:经纤维支气管镜盐酸氨溴索灌洗是治疗支原体肺炎并肺不张疗效显著并降低治疗成本的有效方法。Objective：To evaluate the effecacy and economic benefit of mucosolvan in the treatment of pulmonary atelectasis caused by mycoplasmal pneumonia via bronchoscopy. Methed：93 mycoplasrnal pneumonia（MPP） patients with pulmonary atelectasis were divided into 2 groups randomly. There were treatment group （n = 501 and placebo group（n = 43）. All patients were treated with azithromycin. And in the placebo group, normal saline （NS） were infused via bmnchoscopy. And in the treatment groupj mucosolvan diluted by NS were used. Then observed the length of fever, the times of cough per day, hospital stays and treatment costs and the CT manifestations in 2 weeks later. Results： The cough of the 93 patients was more alleviative than that before the bronchoscopy lavaging（ P 〈 0.05 ）. And cough of the treatment group was more better than that of the placebo goup （ 18.07 ± 4.78vs28.07± 3.39） （ P 〈 0.05 ）. The cost of the treatmenl group was less than that of the placebo group（ P 〈 0.05 ）. The efficient rate of the treatment group was better than that of the placebo group（P 〈 0.05 ）. But the length of the fever of the two groups was no significant difference （ P 〉 0.05 ）. Conclusion： Mucosolvan irrigation via brenchoscopy was an effective and economic method for treatment of mycoplasmal pneumonia with pulmonary atelectasis.
