摘  要:内膜增生是引起移植的小血管远期通畅率下降的主要原因 ,机理是血管平滑肌细胞的移行和增生 ,与局部生长因子有关 .通过 8条犬建立颈动脉间置吻合模型 ,观察冻干辐照人脐动脉 (LIHUA)移植后的内膜增生情况 ,并以犬自体股静脉为对照组 .移植血管段在术后 2~ 6周取材 ,采用组织形态学和免疫组化检测 ,观察吻合口通畅血管段内膜增厚、血小板源生长因子 A链 (PDGF A)和增殖细胞核抗原 (PCNA)的表达 .结果显示 :术后 6周对照组的内膜增生明显比试验组严重 .免疫组化检测表明试验组的PDGF A表达较对照组低 ,术后 6周有显著差异 .随着时间变化的PDGF A表达量与PCNA表达量呈正相关 (r =0 .897,P <0 .0 0 1) ,它与内膜增生发展是相符合的 .说明冻干辐照人脐动脉的内膜增生比自体静脉轻微 。The intimal hyperplasia (IH) is the main cause for the failure restenosis of the vascular graft. It has been suggested that these reactions are caused by the production of growth factor from the cells of the vessel wall. The purpose of this study was to investigate the development of IH of lyophilize irradiated human umbilical artery (LIHUA). In a canine carotid artery model,8 dogs received interposed anastomosis with LIHUA (experimental group) or autologous femoral vein (control group). The grafts were removed in 2 to 6 weeks after the grafting procedure and studied with standard histologic techniques and immunohistochemical for measurement of intimal thickness, platelet derived growth factor A chain (PDGF A) expression and proliferating cell nuclear antigen (PCNA) expression. Intimal thickness of LIHUA is significantly smaller than autologous vein after 6 weeks ( P <0.05). Immunocytochemical analysis detected PDGF A expression in the vascular smooth muscle cells (VSMC) at neointimal/media in the graft. The time course of PDGF A expression paralleled that of PCNA expression and correlated well with the development of IH and PDGF A expression of LIHUA is significantly lower than autologous vein ( P <0.05). In this study we suggested that IH of LIHUA is lower than that of the autologous veins and it may be an ideal graft usable for small caliber artery grafting in the field of coronary surgery.
