基质金属蛋白酶-2及其抑制剂在腹股沟疝患者皮肤中的表达及意义

摘　　要：目的探讨基质金属蛋白酶-2(MMP-2)及其组织抑制因子(TIMP-2)基因mRNA与成人原发性腹股沟疝发生发展的关系。方法应用逆转录-聚合酶链锁反应(RT-PCR)方法检测成人原发性腹股沟疝患者皮肤中MMP-2和TIMP-2基因mRNA的表达。结果直疝患者皮肤中MMP-2呈高表达,TIMP-2呈低表达,与对照组相比差异均有统计学意义(t=2.273、2.461,P均<0.05);斜疝患者MMP-2与TIMP-2的表达,与对照组相比差异无统计学意义(t=1.817、1.672,P均>0.05);MMP-2表达与年龄成正相关(r=0.227),TIMP-2表达与年龄呈负相关(r=-0.372);长期吸烟者MMP-2呈高表达,TIMP-2呈低表达,与非长期抽烟者相比有统计学意义(t=2.301、2.293,P均<0.05)。结论 MMP-2高表达及TIMP-2低表达在成人原发性腹股沟直疝的发生发展中起着重要作用,而在腹股沟斜疝的发生发展中不起重要作用。长期抽烟可促使MMP-2的表达增高,TIMP-2的表达降低,是导致腹股沟疝发生的原因之一。随着年龄增大MMP-2的表达增高及TIMP-2的表达降低,也是老年人腹股沟疝发生率增高的原因。Objective To investigate the association between the expressions of MMP-2 and TIMP-2 with the progress the adults with primary inguinal hernia. Methods Reverse transcription-polymerase chain reaction （RT-PCR） was used to assay the expressions of MMP-2 and TIMP-2 in skins of adults with primary inguinal hernias compared with those with no hernias. Results High expressions of MMP-2 and low expressions of TIMP-2 were observed in direct inguinal hernia compared with those of control group （t = 2. 273,2. 461, P all 〈 0.05 ） ; The expression difference of MMP-2 and TIMP-2 in the group of indirect inguinal herni and the control group was not significant （ t = 1. 817,1. 672, P all 〉 0.05 ） ; Expressions of MMP-2 had a positive correlation with age （ r = 0. 227 ）, and Expressions of TIMP-2 had a negative correlation with age（r = -0. 372） ; Compared the patients with those who were with non-long-term smoking, high expressions of MMP-2 and low expressions of TIMP-2 in long-time smoking were significant（t = 2. 301, 2. 293, P all 〈 0.05）. Conclusions The disorder expressions of MMP-2 and TIMP-2 are significantly associated with the adults with primary direct inguinal hernia development, but are not of greatly related with indirect inguinal hernia. Long-term smoking brought high expressions of MMP-2 and low expressions of TIMP-2, which is one of reasons in the development of hernia. The disorder expressions of MMP-2 and TIMP-2 correlated to the age is the reason of an increase of eider patients with morbidity ratio.
