腹膜前间隙腹股沟疝修补术后并发症的临床研究

摘　　要：目的探讨腹膜前间隙腹股沟疝无张力修补术后并发症的原因及防治措施。方法回顾性分析应用腹膜前间隙腹股沟无张力疝修补术治疗225例患者的临床资料,观察随访患者术后并发症发生情况。结果手术时间40～60min,平均(50±5)min,术后24h能下床活动,住院时间4～8d,平均(6.0±1.5)d。全组术后随访时间8～36个月,术后随访出现并发症13例(5.8%),其中尿潴留4例(1.8%),浆液性水肿4例(1.8%),慢性疼痛、异物感3例(1.3%),缺血性睾丸炎和睾丸萎缩2例(0.9%),随访期间未出现复发患者。结论腹膜前间隙腹股沟疝修补术具有安全有效、术后疼痛轻、恢复快、复发率低、并发症少等优点,是理想的疝修补方法,值得在临床大力推广。Objective To investigate the reasons of complication in postoperative preperitoneal inguinal hernia tension-free repair and the prevention measures. Methods A total of 225 patients with preperitoneal space groin tension-free hernia repair were retrospectively analyzed and their complications were observed. Results The operation time lasted 40 to 60 minutes with a mean of （50 ± 5 ） minutes. The patients can have activities out of bed after 24 hours. The hospital stay was 4 to 8 days with a mean of （6.0 ± 1.5） days. There were 13 cases （5.8%） of postoperative follow-ups for various complications including 4 cases of seroma （ 1.8% ）, 4 cases of urinary retention（ 1.8% ）, 3 cases of chronic pain a foreign body sensation （ 1.3% ）, 2 cases of ischemic orchitis and testicular atrophy （0.9%）. The whole group was followed up for 8 to 36 months without any recurrence. Conclusions It is safe, less invasive with less pain and rapid postoperative recovery for the inguinal hernia tension-free repair with preperitoneal space.
