糖尿病合并心力衰竭的中西医结合防治思路

摘　　要：糖尿病的发病率逐年增加,伴随而来的是冠心病、心脏微血管病变、心脏自主神经病变等患病人数的增加及心功能障碍发生率的升高;与非糖尿病相比,糖尿病存在更多的危险因素导致心力衰竭,预后也较差,致残率、病死率高。糖尿病合并心力衰竭的发病机理复杂,目前没有令人满意的防治策略,西药的不良反应使其临床应用受到一定的限制。中医药在中医理论的指导下,具有多方面、多层次的调节作用。因此,采用中西医结合对预防和延缓糖尿病并心力衰竭的发生及进展可发挥优势互补的作用。With the elevation of living standard,the change of lifestyle and the population becoming aged,the incidence of diabetes mellitus（DM） is increasing yearly,followed with the rising number of the patients with coronary macrovascular disease,microvascular complication,and cardiac autonomic neuropathy.Therefore,patients with diabetic cardiopathy may often have abnormal ventricular systolic and diastolic function,which,in turn,contribute to abnormal myocardial relaxation and elevated ventricular filling pressure.Moreover,diabetic patients were more likely to suffer a myocardial infarction（MI）,had a larger infarct size,and suffered more postinfarct complications,than nondiabetics.As a result,diabetic patients had an increased risk of developing clinical heart failure（HF）,which likely contributed to their excessive cardiovascular morbidity and mortality.The pathogenesis of diabetic HF was so complicated that there was no satisfactory prevention and treatment strategy.At present,the lack of available efficient western medicine drugs for diabetic HF treatment can still be seen in clinic,due to their side effect and toxicity.Traditional Chinese medicine（TCM）,under the guidance of traditional Chinese medicine theory,has superiority of multilevel and multifunction regulation.So the TCM and Western medicine may take advantage of the other to prevent and suspend the occurrence and progression of diabetic HF.Here,the strategy of Integrated traditional Chinese and Western medicine was conducted.
