磁共振成像在乳腺癌的诊断应用价值

摘　　要：目的评价MRI在乳腺癌临床诊断中的应用价值。方法回顾性分析12例经手术或活检病理证实为乳腺癌的MRI特征。结果 12例乳腺癌患者共检出18个病灶,包括4例多灶性乳腺癌和8例单发病灶。1例出现胸肌浸润,8例合并腋窝淋巴结转移。病例分型中浸润性导管癌10例,浸润性小叶癌1例,粘液腺癌1例。病灶边界不清晰,部分可见分叶和毛刺征。T2WI7例呈不均匀高信号,5例呈等、低信号,DWI均为高信号,ADC值在(0.905±0.250)×10-3范围。动态增强扫描病灶8例时间信号强度曲线(TIC)呈"流出型"Ⅲ型曲线,4例呈"平台型"Ⅱ型曲线。2例行MRS检查,1例胆碱峰明显升高,1例轻度升高。结论乳腺MRI对乳腺癌的检出,评估肿瘤侵犯范围以及腋窝淋巴结有无转移具有重要价值。Objective To evaluate the application value of MRI in the diagnosis of breast cancer. Methods Imaging teamres or lz cases with breast cancer confirmed by pathology were analyzed retrospectively. Results A total of 18 lesions was detected in 12 cases, which included multifocal （n = 4） and single （n = 8）. Involvement ofpectoralis muscle was seen in one case, and axillary nodes metastases in 8 cases. Pathological classification： infiltrating ductal carcinoma in 10 cases, invasive lobular carcinoma in one case, and mucous adenocarcinoma in one case. The boundaries of lesions were not clear. Speculate appearance and irregular shape were seen in some cases. MRI manifested as hyperintense on T2W1 in 7 cases and a slight hypo to isointense signal intensity in 5 cases. All the cases had hyper-intensity in DWI, ADC values were in the range of （0.905 ± 0.250） × 10-3. The type of time-signal intensity curves （TIC） analyzed by dynamic enhancement showed type II （platform type） in 4 cases （33.3%）, type III （flow type） in 8 cases （66.6%）. 2 cases also underwent MRS which showed Cho peak severely elevated in one case and slightly elevated in another. Conclusions MRI has great value in detection of lesions, as well as evaluation of tumor invasion and the metastases of the axillary nodes.
