桥接组合式内固定系统对骨折端血供的影响

摘　　要：[目的]观察桥接组合式内固定系统对骨折端血供的影响。[方法]对20条家犬两侧胫骨中段横型截骨,随机选择一侧以桥接组合式内固定系统固定,对侧以普通接骨钢板固定作为对照,分别于内固定后4、8、12、16周行大体病理、HE染色、Ⅷ因子相关抗原免疫组化染色观察并测定微血管密度(MVD)。[结果]内固定后1、3周对照组分别出现骨折劈裂、钢板松脱和钢板断裂各1例,实验组术后5周出现内固定弯曲变形1例。HE染色观察显示:实验组纤维母细胞、新生毛细血管及骨痂的生长均早于对照组,各时间点评分均高于对照组,8、16周时差异有显著性意义(P<0.05)。实验组骨痂的新生血管密度(MVD)均高于对照组,8、12周时实验组与对照组有显著性差异(P<0.05)。[结论]桥接组合式内固定系统在有效提供骨折端稳定、避免应力遮挡的同时,能显著减少对骨折端血供的破坏。[ Objective] To investigate the influence and mechanism of Ortho-Bridger system on fracture blood circulation in dogs. [ Method] Twenty adult canines were osteotomized in the tibia and fixed with Ortho-Bridger system （experimental group） on lefi or fight side at random, another side fixed with plate （control group）. Five animals in each group were sacrificed at 4, 8,12,16 weeks after operation and observed with pathological examination, light microscopy, factor VIII-related antigen immu- nohistochemieal staining for microvessel density（MVD）. [ Result] In control group, split of fracture was found in 1 case at 1 week, broken of plate was found in 1 case at 3 week. While in experimental group, Ortho-Bridger system was flexural deforma- tion in 1 case. Light microscopy showed that the quantity and quality of fibroblast, capillary vessels and callus of experimental group were better than in control group. The HE-stained score of experimental group were higher than in control group, the difference were significant at 8 and 16 week （P 〈 0.05 ）. The MVD score of experimental group were higher than in control group, the difference were significant at 8 and 12 week（ P 〈 0.05 ）. [ Conclusion] The Ortho-Bridger system not only afford firm fixation for fracture fragment, but also reduce the breakage of vessels near fracture part.
