不同促排方案在卵巢低反应患者中的应用比较

摘　　要：目的:通过分析我院卵巢低反应患者行不同促排卵方案的治疗结局,为低反应患者寻找一种最佳的治疗方案。方法:回顾分析2009年1月至2010年12月我院行IVF/ICSI助孕的低反应患者452例。根据治疗方案将患者分为4组:第1组(改良超长方案组)94例,第2组(长方案组)40例,第3组(短方案组)170例,第4组(拮抗剂组)148例。比较各组患者的治疗结局。结果:改良超长方案组患者的HCG日子宫内膜厚度、受精率、着床率均显著高于其他3组(P<0.05);而流产率显著低于其他3组(P<0.05)。改良超长方案与长方案组的临床妊娠率、抱婴率均显著高于其他2组(P<0.05)。结论:对于卵巢储备功能下降的低反应患者,采用改良超长方案助孕可以获得可观的等同于长方案的临床妊娠率和抱婴率,是值得推荐的一种治疗方案。Objective： To find the best protocol for poor response patients, by comparing of various protocols＇outcomes in our hospital. Methods： Retrospectively analysing clinical data of 452 poor response patients who were conducted IVF-ET/ICSI with various protocols in our hospital from Jan. 2009 to Dec. 2010. All patients were divided into 4 groups according to the different treatment protocols：94 patients in the group of modified ultr-long GnRHa protocol （group I ）, 40 patients in the group of long protocol （ group 2）, 170 patients in the group of short protocol（ group 3 ）, 148 patients in the group of antagonist protocol （group 4 ）. Results： The endometrial thickness on HCG adminstration day, fertilization rate and implantation rate of the patients in the group of modified ultr-long GnRHa protocol were significantly higher than the other three groups（P〈0.05 ）. The miscarriage rate was significantly lower than the other three groups （ P〈0.05 ）. The pregnancy rate and birth rate in the groups of modified ultr-long GnRHa protocol and long protocol was significantly higher than the other two groups （P〈0.05）. Con- clusions： For poor response patients of low ovarian col is a new effective treatment method,which can and birth rate as high as long protocols. reserve, the modified ultr-long GnRHa protobe get the satisfactive clinical pregnancy rate
