摘 要:目的探讨再生障碍性贫血(AA)患者外周血白介素-17(IL-17)、干扰素(IFN)-γ水平及T淋巴细胞亚群表达及其在AA发病机制中的作用。方法采用流式细胞仪检测28例AA患者[初治11例,复治17例;急性AA(AAA)7例,慢性AA(CAA)21例]和15例正常对照组T淋巴细胞表达,以酶联免疫吸附法测定IL-17、IFN-γ水平。结果 (1)AA患者外周血IL-17、IFN-γ的水平显著高于对照组(P<0.05),初治组高于复治组(P<0.05),复治组与对照组间比较,差异无统计学意义(P>0.05);IFN-γ水平AAA高于CAA(P<0.05),IL-17水平AAA与CAA比较差异无统计学意义(P>0.05)。(2)AA患者外周血CD4+、CD4+/CD8+、NK细胞的比例显著低于对照组(P<0.05),CD8+的比例显著增高(P<0.05),AAA与CAA之间的差异有统计学意义(P<0.05)。(3)CD4+、CD4+/CD8+、IL-17与IFN-γ与血象呈负相关关系(P<0.05),CD8+与血象呈正相关关系(P<0.05)。结论 AA患者存在不同程度免疫异常,可能与疾病状态有关,多种因素参与AA的发病机制。联合检测免疫细胞及细胞因子可能有助于评估AA病情、疗效及预后。Objective To explore the expression of interleukin-17 ( IL-17 ), interferon-γ( IFN-7), T lymphocyte subsets in peripheral blood of patients with aplastic anemia(AA ) and determine the role of these T cell subsets and cytokines in the pathogenesis of AA. Methods How cytometry was used to measure the expression of T lymphocyte in 28 patients(ll initial treatment cases,17 re-treatment cases;7 cases of acute AA(AAA) ,21 cases of chronic AA(CAA) ) and 15 normal controls, and the levels of IL-17,INF-γ were detected by enzyme-linked immunosorbent assay(ELISA). Results (1) IL-17,IFN-γ elevated markedly in peripheral blood of AA patients compared with the normal controls(P 〈0.05),which were higher in the initial treatment group compared with those in the re-treatment group(P 〈0.05) ,and no significant difference was found in the levels of IL-17 and INF-γ between re-treatment group and normal controls(P 〉0.05) ;The level of INF-γ of AAA patients was higher than that of CAA patients( P 〈 0.01 ), and no significant difference was found in the level of IL-17 between AAA patients and CAA patients (P 〉 0.05 ). (2) The serum CD4+ , CD4 +/CD8 + and NK ceils levels in AA patients were significantly lower than those in the normal controls (P 〈 0. 05), and the CD8+ level increased significantly (P 〈0.05),significant difference was found between AAA and CAA(P 〈0.05). (3) The level of hemogram was correlated negatively with the levels of CD4+ ,CD4+/CD8+ ,IL-17 and IFN-7(P 〈0. 05) but correlated positively with the level of CD8 + (P 〈0. 05 ). Conclusion There are immune disorders of varying degrees in AA patients, which may be associated with the disease state, and many factors involve in the pathogenesis of AA. Determination of immune cells and cytokines expression in AA patients may contribute to the assessment of AA disease,curative effect and prognosis.
