左炔诺孕酮宫内缓释系统对子宫内膜息肉ER、PR表达的影响

摘　　要：目的观察左炔诺孕酮宫内缓释系统对子宫内膜息肉雌激素受体(ER)、孕激素受体(PR)表达的影响,探讨其预防子宫内膜息肉复发的机制。方法经宫腔镜检查、术后病理证实为子宫内膜息肉的患者75例,随机分为两组:观察组41例,宫腔镜下内膜息肉摘除术后放置左炔诺孕酮宫内缓释系统;对照组34例,宫腔镜下内膜息肉摘除术后于月经后半周期口服黄体酮10 d。治疗6个月后,比较两组患者子宫内膜息肉的复发率,ER、PR在子宫内膜(或息肉)的表达及生殖激素的变化。结果治疗后观察组子宫内膜息肉复发率较对照组低(P<0.05),观察组子宫内膜ER、PR的表达显著下调(P<0.05),对照组ER、PR的表达无显著变化(P>0.05),两组患者生殖激素均不受影响(P>0.05)。结论左炔诺孕酮宫内缓释系统可能通过调节ER、PR的表达而预防子宫内膜息肉复发。Objective To observe the effects of levonorgestrel-releasing intrauterine system（LNG-IUS） on the expression of estrogen receptor（ER） and progesterone receptor（PR） in endometrial polyps,and to explore the mechanism of preventing endometrial polyps recurrence. Methods Seventy-five women underwent hysteroscopy and confirmed by postoperative pathology were randomly divided into two groups, 41 cases in the observation group had a LNG-IUS insertion after hysteroscopic polypectomy ,34 cases in the control group were treated with oral progesterone for 10 days in the half cycle of menstruation after hysteroscopic polypectomy. The recurrence rates of endometrial polyps, the expressions of EP, PR in the endometrium （ or polyps） and the changes of reproductive hormones were compared between two groups after 6-month treatment. Results After treatment, the observation group had a lower recurrence rate of endometrial polyps compared with the control group（ P 〈 0.05 ） ,the expressions of ER,PR in the endometrium increased significantly in the observation group（ P 〈 0.05 ）, which showed no significant changes in the control group （ P 〉 0.05 ）, and the reproductive hormons were not affected in both groups （P 〉 0.05 ）. Conclusion LNG-IUS might regulate the expressions of ER, PR to prevent the recurrence of endometrial polyps.
