股骨后髁角CT测量和实体测量的比较研究

摘　　要：目的采用CT测量和实体测量方法对膝关节置换术患者的股骨后髁角进行测量并探讨其临床意义。方法选取40例择期、初次、单侧因骨性关节炎行膝关节表面置换术患者作为研究对象,其中男9例,女31例;内翻屈曲畸形35例(内翻≤15°,屈曲≤15°),无内外翻畸形5例,年龄56～81岁,平均68.3岁。所选病例术前用CT图像测量股骨后髁角,术中用股骨后髁角测量仪测量,算出后髁角。结果测量仪测得后髁角为(3.50°±0.85°);CT测得为(3.26°±0.98°);两种测量方法所得结果之间差异没有统计学意义(t=1.192,P>0.05);个体术前CT测量值和实体测量值之间差异范围0°～0.9°,平均0.41°,说明两种测量方法的结果可以相互印证。结论术前CT测量股骨后髁角是准确的,有助于股骨假体正确安装。Objective To measure the posterior condylar angle of the femur by CT or manually during knee arthroplasty, and explore its clinical significance. Methods 40 patients （nine males and 31 females） with knee osteoarthritis who underwent primary unilateral knee arthroplasty were included in the study. The average age was 68.3 years （56 to 81 years old） old. There was five patients without varus deformity or flexion deformity and 35 patients with varus deformity or flexion deformity （varus deformity≤15°, and flexion deformity≤15°）. The posterior condylar angle was measured on the CT imaging before surgery and was manually measured during surgery by a measuring apparatus. Results The mean posterior condylar angle was （3.50°±0.85°） according to manual measurement during surgery, and it was （3.26°±0.98°） according to CT measurement before surgery. There was no significantly difference between these two values （t=1.192,P〉0.05）. The differential between CT measurement and manual measurement during surgery was 0.41° （range, 0° to 0.9°）, which indicated that the two values were consistent and both the results could be confirmed. Conclusions Measuring the posterior condylar angle of the femur by CT before surgery is precise, and it would be helpful for setting the femoral prothesis in the knee arthroplasty.
