摘 要:目的应用micro-CT监测发育性髋关节发育不良(DDH)大鼠髋关节软骨早期退变并建立相应影像与组织病理改变之间的联系。方法建立新生Wistar大鼠DDH动物模型(n=20),设正常对照组(n=12),于生后10天、4周、6周和8周获取两组髋关节标本,分别行基于造影剂Hexabrix320的micro-CT髋关节软骨成像及组织学蕃红O染色检测,计算股骨头软骨层造影剂浓聚度和石蜡切片蕃红O染色光密度值(OD)以反映软骨基质硫酸糖胺聚糖(sGAG)的含量,采用t-Test,单因素方差分析,LSD-t检验和一般线性回归对数据结果进行统计学分析。结果各时间点的模型组动物关节囊均较对照组明显增厚,伴不同程度的髋关节脱位,生后4周出现髋臼变浅,6周和8周时股骨头变扁,髋臼上方形成假臼;模型组和对照组股骨头软骨层的micro-CT造影剂浓聚度均随时间推移进行性升高(模型组:F=94.265,P<0.01;对照组:F=108.168,P<0.01),但模型组造影剂浓聚度从6到8周时仍显著升高(P<0.05),而对照组后两时间点间的差异无统计学意义(P>0.05);各时间点模型组的造影剂浓聚度均明显高于对照组(10d:t=5.043;4周:t=5.899;6周:t=5.137;8周:t=9.781,所有P<0.01);蕃红O染色OD值与micro-CT造影剂浓聚度变化趋势相反,且二者呈显著负相关(r=-0.867,P<0.01)。结论 micro-CT软骨成像技术能反映髋关节软骨的退变程度,可发现DDH髋关节软骨的早期退变。Objective To detect early cartilage degeneration of developmental dysplasia of hip (DDH) in a rat model by micro-CT (Computed Tomography). Methods 20 newborn Wistar rats were set as DDH model animals by straight-leg swaddling, with other 12 rats as control animals. Hip joints were dissected at the end of 10 days and 4, 6, and 8 weeks after birth, which were subsequently scanned by micro-CT after immersing with Hexabrix 320, and paraffin sections of the specimens were stained with safranin O. The X-ray attenuation in cartilage and the optical density (OD) of safranin O staining were both calculated to quantify the sulphated glycosaminoglycans (sGAG) in the femoral head cartilage. Student's t-Test, one-way ANOVA and general linear regression model were used for statistics analysis. Results Hip joint capsule thickening was seen at all time points with dislocation of the femoral head. Shallow acetabulum at 4 weeks as well as flat femoral heads and formation of pseudo-acetabulum in both 6 weeks and 8 weeks were observed, by micro-CT three-dimensional reconstruction. The cartilage attenuation of the femoral head increased with time gradually in both model and control groups (model: F=94.265, P〈0.01, control: F=108.168, P〈0.01). However, the contrast agent concentration increased significantly from 6 weeks to 8 weeks in model group(P〈0.05) but not in control group(P〈0.05). There was remarkably higher cartilage attenuation in model group compared with control group (10 d: t=5.043, 4 w: t=5.899, 6 w: t=5.137, 8 w: t=9.781; All P〈0.01). The OD value of the femoral head cartilage section showed an opposite tendency to the cartilage attenuation, and a remarkable negative association (r=-0.867, P〈0.01) was also observed. Conclusions Micro-CT arthrography is capable of detecting early cartilage degeneration in DDH rat models.
