血液净化中心实施责任个体化护理模式的效果评价

摘　　要：目的评价在血液净化中心实施责任个体化护理模式的效果。方法将2012年1—6月实施责任个体化护理模式下70例患者进行的5 184例次血液透析作为观察组,将2011年7—12月在层级全责一体化护理模式下68例患者进行的4 752例次血液透析作为对照组,比较两组患者透析并发症(低血压、高血压、低血糖、肌肉痉挛、失衡综合征、透析器反应)的发生率、患者满意度(输液观察、健康宣教、生活护理、服务态度、穿刺技术)及护理质量(内瘘1次穿刺成功率、透析不适症状发生率、护理缺陷发生率)情况。结果观察组实施责任个体化护理模式后,透析并发症的发生率较对照组相比显著降低(P<0.05),患者满意度及护理质量得到明显提高(P<0.05、P<0.01或P<0.001)。结论在血液净化中心实施责任个体化护理模式可减少透析并发症的发生,提高护理质量及患者对护理的满意度。Objective To evaluate the effect of responsibility-individualized nursing mode in blood purification center. Methods Totally 5 184 times of dialyses from January to June 2012, when the responsibility-individualized nursing mode was implemented, were taken as observation group, and 4 752 times of dialyses from July to December 2011, when the nursing mode of hierarchy responsibility-integrated was implemented, as control group. Incidences of dialysis complications （hypotension, hypertension, hypoglycemia, muscle cramps, disequilibrium syndrome, and dialyzer reaction）, patients satisfaction degree （transfusion observation, health education, life care, service attitude, and puncture technique）, and the nursing quality were compared and evaluated between the two groups. Results Since the implementation of responsibility-individualized nursing for six months, the incidences of dialysis complications decreased significantly （P〈0.05）, and patients were more satisfied meanwhile nursing quality had been significantly improved （P〈0.05, P〈0.01, or P〈0.001）. Conclusion The implementation of responsibility-individualized nursing mode could reduce the incidence of dialysis complications, and improve the nursing quality and satisfaction of patients.
