摘  要:目的探讨百草枯中毒存活者肺部影像学(X射线DR和CT)的动态变化。方法从病案室检索2010年1月~2012年11月我院确诊为百草枯中毒,且经规范治疗后随访存活1个月以上的患者,按病程分为早期(≤3天)、中期(3~14天)、中后期(14~28天),以及长期(病程分别≥1个月和≥2个月),回顾性分析患者入院时、住院治疗过程中和出院后至少1个月以上的各时期胸部影像学的动态表现。结果纳入符合条件的4例患者,3例在病程早期有影像学检查结果,其中1例胸部X线片表现为阴性,2例胸部CT表现阴性;中期4例患者均以肺的渗出为主要病灶,随着病程进展,渗出病灶逐渐吸收,以肺的实变及纤维化改变为主。4例患者在中后期均出现肺纤维化,后期3例患者纤维化病灶较前有所吸收。结论影像学检查在百草枯中毒导致的化学性肺炎治疗过程中有重要作用,可通过观察患者肺内病灶形态及范围等情况,了解病情变化,指导疾病的治疗和估计预后。Objective To investigate the lung image survival of paraquat poisoning of science (X-ray DR and CT) dynamic changes. Methods The search for January 2010 to November 2012 in our hospital,diagnosed with pa- raquat poisoning from the record room,and the specification of follow-up after treatment in patients surviving more than 1 months,divided into early according to the course of the disease (≤3 days), middle (3- 14 days), middle (14-28 days),and long-term (the course was more than 1 months and 2 months respectively),a retrospective a- nalysis of the admission process,hospitalization and at least 1 month after discharge of the above period chest radio- graphic dynamic performance. Results A total of 4 eligible patients,3 cases at the early stage with imaging find- ings,including 1 cases of chest X-ray were negative, 2 cases of chest CT manifestations of 4 cases were negative; middle lung exudation as the main focus,as the disease progresses, exudative lesions gradually absorbed,changes in real variable and fibrosis of lung. 4 patients in the middle and late stage were pulmonary fibrosis, 3 cases of patients with late fibrotic lesions than before to absorb. Conclusion Imaging examination plays an important role in pa- raquat poisoning treatment caused by ehemical pneumonia,by observing the shape and range of pulmonary lesions, to understand changes in condition,treatment and estimating prognosis in guiding disease.
