早期整形手术对老年烧伤合并神经损伤患者的效果观察

摘　　要：目的探讨早期整形手术对老年烧伤合并神经损伤患者的临床效果。方法将本院2010年6月～2012年6月收治的40例深Ⅱ度烧伤合并神经损伤的老年患者按照治疗方法分为早期手术组与晚期手术组,每组20例,早期手术组患者于伤后5d进行一期手术治疗,患者移植皮瓣成活半个月内进行损伤神经游离移植手术,对于神经离断患者移植皮瓣成活1个月内行二期神经吻合术。晚期手术组患者于伤后7d进行一期手术,患者移植皮瓣成活1个月内进行损伤神经游离移植手术,对于神经离断患者移植皮瓣成活3个月内行二期神经吻合术,方法同早期手术组。结果术后对两组患者进行6个月的随访,早期手术组感觉功能明显改善率为70.0%,部分改善率为25.0%,无效率为5.0%;晚期手术组明显改善率、部分改善率及无效率分别为45.0%、35.0%、20.0%;两组比较,差异有统计学意义(P<0.05)。结论对老年烧伤合并神经损伤患者进行早期整形手术临床效果显著,患者感觉功能恢复情况优于晚期手术,值得临床推广应用。Objective To study the clinical effect of early surgery for elderly patients with burns complicated neuro- logical damage. Methods 40 cases of elderly patients with deep Ⅱburn combined nerve damage in our hospital from June 2010 to June 2012 were selected, they were divided into early surgical group and late surgical group according to treatment methods, with 20 cases in each group, the patients in early surgical group were given first phase surgery treatment in 5 d after injury, and given injury nerve transplant when transplant flap was survival for half a month, and then given nerve anastomosis in 1 month when transplant flap was survival. Patients in late surgical group were treated with the same method with early surgical group, but the treatment were given in 7 d after injury, in 1 month when transplant flap was survival, in 3 months when transplant flap was survival. Results Patients of two groups were given 6 months followed-up after treatment, obvious improvement rate of sensory function in early surgical group was 70.0%, the part improvement rate was 25.0%, ineffective rate was 5.0%; and the index of late surgical group were 45.0%, 35.0%, 20.0%; the difference between two groups was statistically significant （P 〈 0.05）. Conclusion The clinical ef- fect of early surgical in elderly patients with burns and nerve injury is remarkable, sensory function recovery in pa- tients is better than late surgery, it is worthy of clinical popularization and application.
