摘  要:目的评估术中实时超声辅助下显微切除脑内小型病变的效果,探讨术中超声的临床应用价值。方法 14例海绵状血管畸形、4例胶质细胞瘤,其最大直径均小于3 cm。开颅后,在病变切除前超声寻找最佳入路,切除中明确切除范围,切除后观察是否残留。结果 18例病变术中定位准确,13例非功能区病变均全切,术后无神经功能废损;5例功能区病变全切,术后2例瘫痪加重,随访半年恢复至术前水平。结论术中超声可准确定位,指导手术入路,提高全切率及手术疗效,降低术后并发症。Objective To evaluate the effects of microsurgical resection of intracerebral small space occupying lesion with the help of real-time intraoperative ultrasound(IOUS),and to explore the clinical application value of IOUS.Methods There were 18 cases involved in this study,including 14 ones of cavernous malformations and 4 ones of gliocytoma with the maximum diameter of less than 3 cm.After the craniotomy,IOUS was used to locate the lesions and select the best surgical approach before the resection.During the resection,the resection range and lesion residues were identified by IOUS.Results All the lesions were accurately located.All lesions in the non-functional areas of 13 cases were resected,and no neurological dysfunction occurred.The lesions in the functional areas of 5 cases were resected,but after the operation,hemiplegia occurred in 2 cases that were recovered to the preoperation level during half-year's follow-up.Conclusion The intraoperative ultrasound can accurately locate small intracerebral lesions.It is helpful to select the proper surgical approach,increase the total resection rate,and reduce the incidence of postoperative complications.
