我院门诊2000张中药饮片处方分析

摘　　要：目的:通过对我院门诊中药饮片处方分析,了解我院中药处方的开具和中药饮片在临床使用中存在的问题,促进中药饮片的安全、规范的应用。方法:随机抽取我院2012年12月份的门诊中药饮片处方2000张,从中挑选出不规范处方,进行归类并统计分析。结果:有98张处方不规范,不规范率4.90%,存在配伍禁忌、脚注错误、毒性药超量、诊断与用药不符、剂量错误、不规范修改、用法错误问题。结论:要加强中药处方的管理,增进药师与医师的交流沟通,以促进中药的临床规范应用。Objective:By analyzing outpatient prescriptions of Chinese medicinal decoction pieces in our hospital,to learn about the problems on writing and Clinical application of outpatient prescriptions of Chinese medicinal decoction pieces in our hospital.Methods:2000 outpatient prescriptions of Chinese medicinal decoction pieces in our hospital were randomly collected on December for classification and statistical analysis.Results:There are 98 Unqualified prescriptions.percentage of rejects is 4.90%.There are problems in prohibited combination,prescription footnotes,over-dosage of toxic drug,Diagnosis and medication discrepancies,Dose errors,Not standard revision,Usage problems,dose amount.Conclusion:To improve the management of outpatient prescriptions of Chinese medicinal decoction pieces,to promote the exchange of pharmacists and physicians,for promoting the reasonable clinical application.
