腹腔镜辅助阴式大子宫切除术和经阴道非脱垂大子宫切除术的临床研究

摘　　要：目的探讨腹腔镜辅助阴式大子宫切除术(LAVH)的临床价值。方法回顾2010年1月～2012年9月期间行腹腔镜辅助阴式大子宫切除术49例(LAVH组)临床资料,和同期经阴道非脱垂大子宫切除术37例(TVH组)临床资料进行比较。结果 2组平均手术时间比较:子宫在10～12孕周大小时LAVH组明显长于TVH组(P<0.05);子宫大于12孕周时LAVH组明显短于TVH组(P<0.05)。术中平均出血量比较:子宫在10～12孕周时2组无统计学差异(P>0.05);子宫大于12孕周时,LAVH组明显少于TVH组(P<0.05)。术后肛门排气时间、下床活动时间、术后住院时间、术后体温恢复时间2组差异无统计学意义(P值均大于0.05)。结论 LAVH与TVH均为微创手术,而LAVH行大子宫切除术适应证更广、更容易操作、恢复快、更加安全,值得临床推广。Objective To explore the clinical value of laparoscopic assisted vaginal hysterectomy（LAVH） for large uterus.Methods Clinical data of 49 cases who underwent LAVH for large uterus（group LAVH） between January 2010 and September 2012 were retrospectively analyzed and compared with 37 cases who underwent laparoscopic assisted transvaginal hysterectomy for non-prolapsed large uterus in the same period（group TVH）.Results The average operation time for the uterus in 10 ～ 12 gestational weeks in the LAVH group was significantly longer than that in group TVH（P 0.05）.The average operation time for uterus in LAVH group in over 12 gestational weeks was significantly shorter than that in group TVH（P 0.05）.As regards with the mean intraoperative bleeding,there was no significant difference between the two groups（P 0.05） in 10 ～ 12 gestational weeks.There was obviously less uterus bleeding in over 12 gestational weeks in group LAVH than in group TVH（P 0.05）.There was no statistical difference in terms of anal exhaust time,ambulation time,postoperative hospital stay and postoperative recovery time of body temperature between the two groups（P 0.05）.Conclusion LAVH and TVH are both minimally invasive operation,but LAVH for large uterus is characteristic of wider indications,easier operation,quicker recovery and more safety,which is worth to be recommended.
