雷替曲塞单药治疗老年晚期结直肠癌的临床疗效观察

摘　　要：目的评价雷替曲塞单药治疗老年晚期结直肠癌患者的疗效和安全性。方法对62例老年晚期结直肠癌患者随机分为2组:治疗组30例患者采用雷替曲塞单药治疗;对照组32例患者采用5-氟尿嘧啶/亚叶酸钙(5-FU/LV)治疗,对2组临床疗效进行回顾性比较分析。至少进行2个周期,评价治疗的近期疗效和不良反应,随访至疾病进展时间(TTP)。结果治疗组患者CR 2例,PR 7例,SD 9例,PD 12例,总有效率RR为30.0%,疾病控制率DCR为60.0%;对照组患者CR 0例,PR 6例,SD 10例,PD 16例,总有效率RR为18.7%,疾病控制率DCR为50.0%;2组比较差异无显著性(P>0.05)。2组主要不良反应为恶心、呕吐、转氨酶升高和中性粒细胞减少以及静脉炎,多为轻中度,2组差异有显著性(P<0.05)。结论雷替曲塞单药方案是老年晚期结直癌有效的姑息治疗方案,不良反应少,生存期方面优于或不劣于5-FU/CF单药化疗,值得在临床上推广应用。Objective To evaluate the therapeutic effect and safety of raltitrexed in treatment of elderly patients with advanced colorectal cancer.Methods 62 cases of elderly patients with advanced colorectal cancer patients were randomly divided into two groups： a treatment group of 30 patients treated with single raltitrexed and a control group of 32 patients treated with 5-fluorouracil / leucovorin calcium（5-FU / LV）.The clinical effect of the two groups were retrospectively compared and analyzed.The comparison and analysis lasted at least two cycles so as to evaluate the short-term efficacy and adverse reactions.Follow-up was carried out until time to progression（TTP）.Results In the treatment group,there were 2 cases of CR,7 cases of PR,9 cases of SD and 12cases of PD,with total efficiency RR at 30.0% and disease control rate（DCR） at 60.0%.In contrast,there was 0 case of CR in the control group,6 cases of PR,10 cases of SD and 16 cases of PD.The total efficiency RR was 18.7% and disease control rate（DCR） 50.0%.The difference was not significant（P 0.05）.The major adverse reactions in both the two groups were nausea,vomiting,elevated transaminases,neutropenia as well as phlebitis,mostly mild to moderate in degree.The difference between the two groups was significant（P 0.05）.Conclusion Raltitrexed monotherapy is a clinically effective palliative therapy for elderly patients with advanced colorectal cancer.It is of many advantages such as having fewer adverse reactions and longer survival span.Its effect is better than or superior to that achieved by the single drug chemotherapy of 5-FU / CF.It is worth to be recommended in clinical application.
