槐杞黄颗粒联合参芎葡萄糖治疗难治性肾病综合征的临床研究

摘　　要：目的观察槐杞黄颗粒联合参芎葡萄糖注射液治疗难治性原发性肾病综合征的临床疗效。方法将60例难治性原发性肾病综合征患者随机分为两组,每组30例。治疗组给予槐杞黄颗粒10g,1天2次口服,同时给予参芎葡萄糖注射液200ml,每日一次静脉滴注;对照组给予参芎葡萄糖注射液200ml,每日一次静脉滴注;两组均给予泼尼松1mg.kg-1.d-1口服治疗。于治疗后1个月分别测定两组患者24小时尿蛋白定量、血浆白蛋白(ALB)、甲状腺结合球蛋白(TBG)、总三碘甲状腺原氨酸(TT3)、反T3(γT3)及全血黏度、血浆黏度、纤维蛋白原(Fib)、凝血酶原时间(PT)、血小板计数水平(PLT)。结果 1个月后治疗组24小时尿蛋白定量水平低于治疗前及同期对照组水平(P<0.05),ALB、TBG、TT3、γT3高于治疗前及同期对照组(P<0.05)。治疗1个月后与治疗前比较,两组患者全血黏度、血浆黏度、Fib、PLT水平下降,而PT延长(P<0.05)。结论槐杞黄颗粒联合参芎葡萄糖注射液治疗难治性原发性肾病综合征不仅能减少该类患者的蛋白尿,纠正甲状腺素水平紊乱,还能改善肾病综合征的血液高凝状态。Objective To observe the clinical effect of Huaiqihuang granule combined with Shenxiong glucose injection on patients with refractory primary nephrotic syndrome. Methods 60 patients diagnosised with refractory primary nephrotie syndrome were randomly divided into the treament group and the control group with 30 cases in each group. The treament group were given 10 g of huaiqihuang granule（for oral administration twice a day） and 200 ml of Shenxiong glucose （for venous injection once a day）. The control group were only treated with 200 ml of Shenx- long glucose（ for venous injection once a day）. Both groups took 1 mg ~ kg-~ ~ d ~ of prednisone orally once a day. Then, the urinary protein quatitation for 24 hours, plasma albumin（ALB）,thyroid binding globulin （TBG）, total triio- dothyronine（TT3）, reverse triiodothyronine（）＇T3 ） as well as the whole blood viscosity, plasma viscosity, fibrinogen （Fib） ,prothrombin time（PT）,and blood platelet count（PLT） of patients in both groups were all measured after one month treament. Results After treating for one month, the treatment group had lower urinary protein quantitation for 24 hours（ P d0.05） and higher levels of ALB,TBG,TT3 and yT3 （ P d0.05） than the control group before treat- ment and at the same period. The whole blood viscosity,plasma viscosity, Fib,and PLT of both groups decreased but their PT increased after treament for a month. Conclusion Huaiqihuang granule combined with Shenxiong glucose in treating refractory primary nephrotic syndrome can not only reduce patients＇ proteinuria and correct the disorder of the thyroid hormone levels, but also improve the hypercoagulable state of nephrotfc syndrome.
