子宫螺旋动脉血流参数结合血清抑制素A在先兆流产中的应用价值

摘　　要：目的应用经阴道彩色多普勒超声(CDFI)监测先兆流产孕妇子宫螺旋动脉(USA)血流参数及血清抑制素A的变化,探讨其临床应用价值。方法选择孕60～7+6周及孕80～9+6周孕妇各90例,分为正常早孕组(A组)、先兆流产保胎成功组(B组)、先兆流产保胎失败组(C组)各30例,应用CDFI检测各组的USA血流参数及血清抑制素A结果进行分析。结果两阶段孕周孕妇C组与同阶段的A组、B组比较,USA血流参数及血清抑制素A差异均有显著性(P<0.01),RI有明显增高,PSV、EDV、抑制素A明显降低(P<0.01);先兆流产患者USA血流参数、血清抑制素A与不良结局有一定相关性;RI结合血清抑制素A预测妊娠结局比单一使用其一的灵敏度及诊断指数要高。结论应用CDFI测定USA血流参数及血清抑制素A预测先兆流产妊娠结局有重要的应用价值。Objective To monitor the changes of blood flow parameters of the uterine spiral artery（USA） and serum inhibin A of pregnant women with threatened bortion by color dopple flow imaging（CDFI） so as to observe the clinical value. Methods Women who were pregnant in the period of 60 to 7＋6 weeks （90 cases） and period of 8o to 9＋6 weeks（90 cases） were respectively divided into the normal early pregnant group（group A）,the group of suc- cessful prevention from threatened bortion（group B）and the group of failing to prevention threatened bortion（group C）with 30 cases in each group. Then, the USA blood flow parameters and serum inhibin A of all the groups were measured by CDFI and the results were analyzed and compared. Results There were significant differences in the USA blood flow parameters and serum inhibin A between group C and group B,and group A in the same period（ P〈0.01）. The result also showed that RI significantly increased,while PSV,EDV and serum inhibin A obviously decreased（ P〈0.01）. The adverse outcome of women with threatened bortion had something to do with their USA blood flow parameters and serum inhibin A of. RI combined with serum inhibin A in predicting pregnant outcome had higher sensitivity and diagnostic index than the application of one of them only. Conclusion The blood flow parameters of USA combined with serum inhibin A tested by CDFI have important value in predicting the pregnant outcome of threatened bortion.
