子痫前期患者妊娠早晚期血清胎盘蛋白13变化的研究

摘　　要：目的探讨胎盘蛋白13(PP13)在子痫前期患者妊娠早晚期血清中的变化及其意义。方法应用酶联免疫吸附试验(ELISA)对169例孕前血压正常的孕妇,在妊娠9～11周、32～34周2个时间段对母体血清中的PP13水平进行测定,观察孕妇孕期病情变化并随访妊娠结局。结果①169例孕妇发展为子痫前期21例,其中8例重度子痫前期、13例轻度子痫前期;148例妊娠结局正常。②169例孕妇血清PP13水平随着妊娠的进展,进行自身配对比较均逐渐上升,妊娠早晚期之间比较,差异有显著性意义(P<0.01)。③在妊娠9～11周,轻度子痫前期组、重度子痫前期组血清PP13水平均显著低于对照组(P<0.01);在妊娠32～34周,轻度子痫前期组、重度子痫前期组血清PP13水平稍高于对照组,但轻度子痫前期组PP13与对照组比较,差异有统计学意义(P<0.05),而重度子痫前期组PP13与对照组比较,差异无统计学意义(P>0.05)。结论子痫前期患者妊娠早期即有血清PP13水平异常表达,可能参与了子痫前期的发生与发展。Objective To determine serum levels of placental protein 13 （PP13）in first and third tri- mester in preeclampsia patients. Methods The prospective study method was used to take an enzyme linked immunosorbent assay （ELISA） for 169 pregnant women with previously normal during their progestation. Pe- ripheral venous blood samples were obtained during 9 - 11,32 - 34 weeks of gestation. The pathogenetic con- dition and pregnancy outcomes of those cases were observed. Results ① Among 169 previously normotensive women,21 developed into preeclampsia, including 8 severe and 13 mild preeclampsia. 148 remained normal till the end of pregnancy.② With the development of pregnancy, the levels of 169 cases whose maternal serum levels of PP13 were gradually ascended, there was significant difference between two different gestationa stages （P 〈0.01 ）. ③ PP13 levels in serum were significantly decreased in women in both mild and severe pre- eclampsia groups as compared with that in normal group during 9- 11 gestational weeks（P 〈0.01 ）. The lev- els of PP13 were higher in both mild and severe preeclampsia groups as compared with that in normal group during 32- 34 gestational weeks, but there were significant differences between mild preeclampsia group and normal group （ P 〈 0.05 ） , there without significant differences between severe preeclampsia group and normal group （P 〉 0.05 ）. Conclusion Expression of serum PPI3 was abnormal in preeclampsia patients in first tri- mester,which indicate PP13 play a role in the occurrence and progress of preeclampsia.
