特异性免疫治疗对变应性鼻炎近期疗效和安全性的临床研究

摘　　要：目的观察变应原特异性免疫治疗(ASIT)对变应性鼻炎(AR)的疗效及安全性。方法选择东莞市人民医院2011年1月-2012年3月经确诊的螨变应原导致的中重度持续性变应性鼻炎患者100例,采用变应原特异性免疫集群治疗方案,每周治疗1次,每次注射2-3次,6周达到维持剂量(观察组);另选择东莞市人民医院往期接受常规免疫治疗的螨变应原导致的中重度持续性变应性鼻炎患者100例作为对照组,治疗期间通过鼻部症状总评分、药物评分评价临床疗效,同时观察治疗导致的全身不良反应以评价治疗的安全性。结果经过1年变应原特异性免疫治疗后,观察组症状改善总分为(4.16±1.94)分,对照组为(7.92±1.33)分,两组比较差异有统计学意义(P<0.05);观察组总有效率为98.0%,对照组总有效率为78.0%,两组患者临床疗效差异有统计学意义(χ2=4.824,P<0.05);两组不良反应发生率比较差异无统计学意义(P>0.05)。结论变应原特异性免疫治疗治疗变应性鼻炎起效快、安全、效果显著,可在临床推广使用。Objective To observe the allergen specific immunotherapy（ASIT） on allergic rhinitis（AR） of the efficacy and safety of.Methods In Dongguan People＇s Hospital from January 2011 to Mach 2012 admitted to hospital,diag nosed by mite allergen in 100 cases of patients with severe persistent allergic rhinitis,the allergen-specific immune clustering scheme for injection,once a week,each 2-3 times injection,6 weeks to reach maintenance dose（observa tion group）;lead to other institute of the routine immunotherapy with mite allergen in patients with severe persistent allergic rhinitis patients 100 patients as control group,treatment during the evaluation of the clinical efficacy of the to tal score,nasal symptoms and medication scores,safety and observe the adverse reaction of whole body treatment lead to evaluate the treatment.Results After 1 years of treatment of allergen-specific immune,the observation group im proved symptoms total score was（4.16±1.94） points,control group was（7.92±1.33） points,there was significant difference between two groups（P〈 0.05）;total effective rate in the observation group was 98.0%,total effective rate of the control group was 78.0%,statistically significance of differences between the two groups of patients with clinical curative effect（χ^2=4.824,P〈 0.05）;the incidence of adverse reactions of the two groups showed no statistical significance（P〈 0.05）.Conclusion Allergen-specific immunotherapy for allergic rhinitis,safe fast onset,obvious effect,can be used in clinical practice.
