超微心电图在诊断小儿房性心律失常中的应用

摘　　要：目的　探讨超微心电图 (SMECG)在诊断小儿房性心律失常中应用价值。方法　对349例疑有心律失常的患儿 ,采用超微心电图仪同步实时描记SMECG和ECG的II、AVR或及AVF导联 ,检测毕描记 9或 12导联ECG。结果　SMECG检出房性心律失常 98例 ,ECG检出 79例 ,余 19例未检出 ,误检或漏检率为 19 4%。两者检验结果经卡方检验 ,P <0 0 0 5 ,有显著差异。结论　SMECG能直接反映窦房结的电活动、窦房间传导情况及心房兴奋的起源与传播等。可提高小儿房性心律失常的检出率。Objective To investigate the significance in application of supermicro-eletrocardiogram(SMECG) in diagnosis of atrial arrhythmia in children.Methods Leads of Ⅱ,AVR and/or AVF traced by electrocardiogram(ECG) and SMECG were simultaneously conducted in 349 children with arrhythmia.Subsequently,ECG of 9 or 12 leads was conducted in the patients.Results In 98 cases of atrial arrhythmia diagnosed by SMECG,79 cases of atrial arrhythmia were also diagnosed by ECG but 19 cases failed.This significant difference was confirmed by analysis of Chi-square(χ 2=16 2, P <0 005).Conclusion SMECG directly reflects the electrical action of sinus nod conduction betwecn sinus and atrius,and origin of atrial excitation.Thus,SMECG raises the detection rate for atrial arrhythmia.
