摘 要:目的:探讨F波和H反射对糖尿病近端神经病的诊断作用。方法:应用F波、H反射和常规周围神经传导检测方法对正常对照组37例及糖尿病组106例进行对比研究。结果:经腿长与年龄调整后,正常人胫神经F波最短潜伏期(FLmin)多元回归方程为5.53+0.04×腿长(cm)+0.08×年龄(岁),正常双侧FLmin侧间差不大于3.3ms。糖尿病组F波异常率为40.1%,H反射异常率为45.3%,感觉和运动神经传导速度异常率分别为40.5%和37.8%。结论:F波和H反射对检测糖尿病近端神经病均具有较高的诊断敏感性,H反射敏感性更高。与远端神经病变相比较,糖尿病近端神经病具有相当高的患病频度,糖尿病近端神经病中纯运动神经病亦有较高的发生频度。Objective:To explore the value of F wave and H reflex in diagnosis of diabetic proximal neu- ropathy.Methods:F wave,H reflex and routine nerve conduction were performed and compared in 106 patients with diabetes mellitus and 37 healthy controls.Results:The regression formula of the minimal F wave latency (FLmin) of tibial nerve on the ages and leg lengths in normal subjects produced as 5.53+0.04x(leg length)+ 0.08x(age).The normal side difference of FLmin is not more than 3.3ins.The abnormal rate of F wave,H re- flex,sensory conduction velocity and motor conduction velocity of tibial nerve of diabetic patients were 40.1%, 45.3%,40.5% and 37.8%,respectively.Conclusion:Both F wave and H reflex were approved high diagnostic sensitivity for detecting diabetic proximal neuropathy,and with higher sensitivity for the H reflex.A high incidence rate of diabetic proximal neuropathy was confirmed to be comparable to that of distal neuropathy, pure proximal motor neuropathy in diabetic patients also had a high frequency.
