摘 要:目的探讨经皮超声造影(CEUS)检测乳腺癌前哨淋巴结(SLN)的应用价值,比较经皮CEUS与常规超声在诊断SLN性质的价值差异。方法建立兔VX2乳腺癌模型26只。行常规超声检查后,皮下注射造影剂SonoVue,通过造影剂在淋巴管中实时显像寻找造影剂到达的第一站淋巴结,即SLN,予以定位,术中单独分离。再进行腋窝淋巴结清除术,根据病理结果评估超声造影探测的SLN是否是临床真正的SLN,分析SLN显像成功率。并将所有腋窝淋巴结分为阳性组(有转移组)和阴性组(无转移组),分析两组淋巴结常规超声表现及经皮CEUS造影模式。结果经手术证实,所有经皮CEUS定义的SLN,都为临床上真正意义的SLN显像成功率为97%(32/33);经皮CEUS对腋窝淋巴结的检出率,以及对淋巴结性质判断的准确率明显高于常规超声;与阳性组相比,阴性组纵横比(L/T)值高,两组间的血流类型、经皮CEUS增强模式差异均有统计学意义(P<0.05)。结论经皮CEUS不仅可以清晰显示SLN引流路径,还可判定性质,较常规超声更有价值。Objective To discuss the prospects of detecting breast cancer SLN by percutaneous CEUS, and to compare tile diagnostic value on detecting breast cancer SLN by CEUS with by the traditional US (ultrasonography). Methods 26 rabbits models with VX2 breast cancer were set up. After examination by traditional US, subcutaneous injection of SonoVue was conducted. The first of lymph nodes were found by positioning the contrast agents through the real time imaging in lymphatic capillary. The SLNs were isolated and dissected during the surgery. Then the axil lary lymph nodes would be eliminated. According to the pathological results, the success rates of ultrasonic imaging were evaluated, and tbe axillary lymph nodes were divided into Positive Group (metastatic) and Negative Group (non- metastatic), and to analyze the contrast model of lymph nodes presented in imaging of two groups by the traditional US and CEUS. Results All pereutaneous CEUS defined SI.Ns which confirmed by surgical operation were clinical true meaning. The success rate of ultrasonic imaging was 97% (32/33). The detection rate and accuracy rate onaxil lary lymph node by Percutaneous CEUS were higher than those by conventional ultrasound. A higher value of L/T ap peared in the negative group, compared with the positive group. Statistically differences between the two groups (P〈0.05) were found in the vascular pattern and the contrast model of percutaneous CEUS. Conclusions The percutane- ous CEUS was more effective, compared with traditional US. Percutaneous CEUS could not only clearly show the SLN drainage path, but also determine the nature of the SLN.
