摘 要:目的:检测冠心病(CHD)患者外周血血浆循环游离线粒体DNA(ccf-mtDNA)含量并初步探讨其临床应用价值。方法:收集106例CHD患者和106例健康志愿者的外周血样本,以荧光实时定量PCR方法检测各组血浆ccf-mtDNA含量,比较分析血浆ccf-mtDNA水平与CHD的临床相关性。结果:CHD患者血浆ccf-mtDNA含量明显高于对照组(P<0.01)。以CHD组血浆ccf-mtDNA含量绘制ROC曲线,曲线下面积为0.724(95%可信区间为0.665~0.783),以血浆ccf-mtDNA含量≥0.71×106为临界值,用ccf-mtDNA诊断CHD的敏感度为74.7%,特异度为73.0%。血浆ccf-mtDNA含量与CHD患者SBP、DBP、TC、LDL-C、FPG显著正相关(P<0.05),与年龄、HDL-C显著负相关(P<0.05)。结论:血浆ccf-mtDNA含量对CHD患者的早期诊断及病程监测具有重要的临床价值,可作为CHD事件发生的监测指标。Objective: To detect the level of the circulating freeing mitochondrial DNA ( ccf - mtDNA) in plasma of patients with coronary heart disease (CHD) and to evaluate its potential clinical application values. Methods: Peripheral blood samples were collected from 106 cases of CHD and 106 cases of healthy controls. The mtDNA con-tent in the peripheral blood was detected by real - time fluorescence quantitative PCR assay, the associativity of ccf - mtDNA content and clinical characteristic of CHD was compared and analyzed. Results: The level of ccf - mtD-NA in CHD group was significantly higher than that of healthy controls (P 〈 0.01 ). ROC curve was drown accord- ing to the eef - mtDNA level, area under curve was 0.724 (95% CI: 0. 665 - 0.783 ). The level of cef - mtDNA ≥0.71 × 10^6 was used as critical value to diagnose CHD, the sensitivity and specificity was 74.7% and 73.0%, respectively. Plasma ccf-mtDNA content was positively associated with SBP, DBP, TC, LDL - C, and FPG, but negatively associated with age and HDL - C ( P 〈 0.05 ). Conclusion: Plasma ccf - mtDNA content play an impor-tant role in early diagnosis of CHD. It can be used as a monitoring indicator for CHD.
