摘 要:目的:探讨导杆式矫治器在矫治上颌非遗传性多个埋伏牙中的作用。方法:5例患者均采用外科手术暴露埋伏牙牙冠,在埋伏牙牙面上粘结舌侧扣并缝合创口;同期戴入导杆式矫治器,利用其一根或多根延伸到唇侧前庭沟导杆末端的附钩,将不同埋伏牙牙冠朝其正常的位置方向牵引,解除埋伏牙间的重叠及萌出干扰,直到接近其正常的位置。采用直丝弓技术排齐埋伏牙,调整咬合关系。检查治疗后牙齿的松动度、牙髓活力。结果:5例上颌非遗传性多个前牙均被牵引至牙弓的正常位置,牙髓活力测试正常,牙龈形态基本正常;无邻牙和埋伏牙牙根吸收。结论:导杆式矫治器在矫治上颌非遗传性多个埋伏牙时,在解除多个牙间萌出时的相互干扰和支抗控制方面有独特优点和作用。Objective: To investigate the effect of the modified nance arch on multiple nonhereditary maxillary impacted teeth. Methods: The combined approach of orthodontics and surgery was performed on 5 cases of multiple nonhereditary maxillary impacted teeth. After surgical exposure, a lingual knot button was firmly bonded on the crowns of impacted teeth. The modified nance arch was placed with a hook or two extended to the labial vestibule to apply traction on the crowns of impacted teeth to move to their ideal positions. During the process of traction, the direction of hook was gradually adjusted in order to allow the unerupted teeth an unobstructed path into the mouth. Once they were fully erupted, suitable tipping, rotational, uprighting, and torquing forces are applied by SAW Technique to the teeth to achieve adequate alignment. The mobility and vitality of the impacted teeth were detected after treatment. Results: Five cases of nonhereditary maxillary anterior teeth were fully erupted to their normal positions, with normal pulp activity and gingival morphology. No root resorption was detected in adjacent teeth and the impacted teeth. Conclusion: The modified nance arch is strategically advantageous for applying different directional orthodontic traction and anchorage control.
