T型钢板内固定治疗胸锁关节脱位78例疗效分析

摘　　要：目的观察T型钢板内固定治疗胸锁关节脱位的疗效及安全性。方法选取胸锁关节脱位患者78例，取患侧胸锁关节前侧入路，沿锁骨内侧弧形切口，沿锁骨近端将锁骨的胸骨端及胸骨行骨膜下部分剥离，选取合适长度T形钢板，松质骨螺钉固定。结果平均手术时间（44±19）min，平均出血量（40±23）ml。临床疗效：优66例，良11例，中1例，优良率为98．7％。脱位关节均达到解剖复位，且钢板及钢钉无松动、断裂等情况。术后未发生血气胸、关节再脱位、切口感染等不良反应。结论T型钢板内固定治疗胸锁关节脱位疗效可靠，安全性高，值得临床推广。Objective To observe the effect and safety of T type plate internal fixed treatment on sternoclavicular joint dislocation. Methods Seventy-eight cases of sternoclavicular joint dislocation were selected, took a side the sternoclavicular articulation of the side into the road, along the clavicle inside arc incision along the clavicle near end will clavicular sternal end and sternum periosteum stripping down part, chose the suitable length T steel, cancellous bone screws. Results The mean operation time was （44.3± 19.2）min, the average amount of bleeding was （39.5 ± 22.7 ） ml. Clinical curative effect： optimal in 66 cases, good in 11 cases, moderate in 1 case, the good and excellent rate was 98.7%. Dislocation joints to anatomy reset, and steel plate and steel nail no looseness, fracture, etc. Hemopneumothorax, joint dislocation, infection of incision or other adverse reactions didn＇ t occur postoperatively. Conclusions The curative effect of T plate internal fixed treatment on sternoclavicular articulation dislocation is reliable, has high safety, so it is worth clinical promotion.
