子宫瘢痕缺陷处理的临床探讨

摘　　要：目的探讨剖宫产等子宫瘢痕缺陷（DUS）经阴道切除及腹腔镜切除的临床疗效。方法对南方医科大学附属中山博爱医院2010年12月至2012年8月采用阴式或者腹腔镜下子宫瘢痕缺陷切除术治疗的剖宫产子宫瘢痕缺陷26例患者的资料进行回顾性分析。结果所有手术顺利完成，腹腔镜组及阴式组年龄、手术时间、术中出血量比较差异无统计学意义（P〉0．05）。术后并发症、术后平均住院日比较差异有统计学意义（P〈0．05）。术后随访4～22个月，症状缓解率为92．3％，术后经期较术前明显减少，差异有统计学意义（P〈0．01）。结论经阴道或经腹腔镜切除子宫瘢痕憩室治疗DUS相对微创，近期效果满意，减少再次妊娠并发症，值得临床推广。Objective To investigate the clinical effect of transvaginal and translaparoscopical excision on patients with defect of uterine scar. Methods The clinical data of twenty-six patients with the defect of uterine scar in Zhongshan Boai hospital affiliated to southern medical university between Dec. 2010 and Aug. 2012 underwent transvaginal or translaparoscopical excision of the uterine scar defect were retrospectively analyzed. Results The procedures were successfully performed in all patients. The average duration of the surgery, average volume of intraoperative bleeding and age had no statistical significance（ P 〉 0. 05）. Post-surgery length of hospital stay, the complication of operation had statistical significance（ P 〈 0.05）. After surgery, the duration of menstruation was shorter in average,which was statistically significant（ P 〈 0.01）. After the clinical following-up of 4-22 months, the total symptom remission rate was 92.3%, the post-menstrual period was obvious decreased than before operation, there were significant differences（ P 〈 0. 01 ）. Conclusions Transvaginal or translaparoscopical excision of the uterine scar defect has satisfactory short-term effect and few complications. It is worth to be popularized in the treatment of patients with cesarean section or myomectomy scar defect.
