阿米替林中毒致室性心律失常1例

摘　　要：1病例资料患者,女,38岁,因＂服药后神志不清5h＂收治入院。患者于当日中午12时左右服下约50片阿米替林（1250mg）后神志不清,无四肢抽搐,无大小便失禁。家人发现后送至当地医院就诊,血化验结果以及心电图结果不详,于14时30分左右予以洗胃,同时行留置导尿、水化、促醒等对症处理,但患者神志未恢复。A 38-years-old female patient was unconscious for 5hours after taking about 1 250mg amitripty- line.And different approches such as gastrolavage,infusion,and wake promoting agent were used in local hospital without working.She was still unconscious with hypopnea when admitting in our hospital.The vital sign was sta- ble.The arterial blood gas showed pH 7.21,pCO262mmHg（1mmHg=0.133kPa）,pO2137mmHg,standard bicarbonate 22.2mmol/L,base excess-3.6mmol/L.The blood routine test,hepatic and renal function,elec- trolyte,CK-MB,TnT and coagulation function were almost normal.The electrocardiography demonstrated accel- erated idioventricular rhythm with approximate 100beats per minute.The ventricular arrhythmia was considered associated with amitriptyline poisoning.
