超声心动图技术多参数评价兔左室舒张功能的探讨

摘  要:目的应用超声心动图技术多参数评价正常新西兰白兔左心室舒张功能,建立正常参考值,比较各种方法的相关性。方法测量左心室舒张功能方法包括脉冲多普勒技术、组织多普勒技术以及彩色多普勒M型法。正常新西兰白兔25只,应用脉冲多普勒测定二尖瓣口舒张早期血流速度(E)、舒张晚期血流速度(A),计算E/A比值,应用组织多普勒技术(DTI)测定舒张早期二尖瓣环室间隔及侧壁运动速度平均值(e’),舒张晚期运动速度平均值(a’),计算e’/a’比值,E/e’比值。应用脉冲多普勒测量等容舒张时间(IVRT)。应用彩色多普勒联合M超测量二尖瓣口舒张期血流传播速度(Vp)。计算各参数正常值。结果25只兔中有3只因为心率J夹导致E、A峰,e’、a’峰融合,故无法获得E/A、e’/a’比值。心率参考范围为180~324次/分,多参数获得正常参考值范围如下:E峰、A峰、E/A比值分别为0.44~1.05m/s、0.23~0.47m/s、1.47~2.67。e’峰、a’峄、e’/a’比值、E/e’比值分别为6.96~19.92cm/s、3.82~12.77cm/s、0.88~2.58、2.70~8.80。IVRT和Vp分别为22.90~58.06ms,38.90—81.52。E/A与e’/a’相关性显著。E/A、e’/a’与IVRT、Vp无统计学意义的相关性。结论脉冲多普勒技术及组织多普勒技术能够在以兔为模型的心血管疾病研究中敏感有效地评价兔左心室功能。Objective To evaluate the left ventricular diastolic function in healthy New Zealand White rabbits by multi-parameter echocardiography. To obtained normal values and analysis their correlation. Methods Left ventricular diastolic function can be measured by Pulsed Doppler, Tissue Doppler imaging and color M-mode Doppler. twenty-five rabbits were used in this study. Valves of mitral flow spectrum (E,A) and E/A were detected by Pulsed Doppler. Early and late diastolic peak velocity of mitral annulus (e',a') and e'/a', E/e' were measured by Tissue Doppler imaging (DTI). Isovolumic relaxation time (IVRT) was measured by Pulsed Doppler. Early diastolic flow propagation velocity (Vp) was measured by color M-mode Doppler. And we obtainednormal values of left ventficular diastolic function. Results The peak of E and A ,the peak e' and a' mixed together in three cases of all. Heart rate range was 180-324 beats/min. The normal values of left ventficular diastolic function were: E=0.44- 1.05 m/s,A=0.23-0.47m/s, E/A=1.47-2.67.e'=6.96-19.92cm/s, a'=3.82-12.77cm/s, e'/a'=0.88-2.58, E/e'=2.70-8.80. IVRT=22.90-58.06 ms, Vp=38.90-81.52 respectively. There were significant correlation between E/A and e'/a'. No statistically significant among E/A, e'/a', IVRT, Vp. Conclusions Left ventricular function of rabbits in cardiovascular disease can be evaluated by Pulsed Doppler and Tissue Doppler imaging.
