摘　　要：目的应用超声影像学技术评估艾滋病(AIDS)患者抗逆转录病毒治疗后左室结构、功能以及腹腔脏器形态学的变化。方法应用超声心动图技术于41例AIDS患者(AIDS组)用药前基线状态、抗病毒用药48周及96周分别测量其心脏形态结构参数,同时应用腹部探头测量AIDS组及38例健康体检者(对照组)的肝脏、脾脏大小及门静脉内径。结果 AIDS组用药96周与用药前、用药48周比较,除左室流出道血流速度、二尖瓣口E峰减速时间均有所提高(P<0.05)外,其余各参数比较差异均无统计学意义;AIDS组用药前的脾脏肿大阳性率较对照组显著增加(χ2=10.94,P<0.05),用药前、用药48周及用药96周3个时间点间的脾脏肿大阳性率比较差异均无统计学意义。结论 AIDS患者脾脏肿大者明显多于健康成人,短期抗病毒治疗不会影响心脏结构功能,且不会加重AIDS患者的脾肿大程度,超声可准确评估抗病毒治疗后AIDS患者心脏结构功能及腹腔脏器形态学变化。Objective To discuss the value of uhrasonography in evaluating the function of left ventricle and abdominal organ＇s morphological changes in AIDS patients after highly active antiretroviral therapy （HAART）. Methods Forty-one HIV- infected patients （AIDS group） accepted three times of echocardiography and abdominal uhrasonography at baseline state before treatment, 48 weeks and 96 weeks after HAART. Abdominal ultrasonography was also applied to measure the size of liver and spleen and the inner diameter of portal vein in 38 healthy control subjects （control group）. Results There were significant differences in V-LVOT and EDT among baseline group,48 weeks HAART group and 96 weeks HAART group （P〈0.05）, the rest echocardiography parameters showed no significant differences, the positive rate of splenomegaly in baseline group was significantly increased compared to that in the control group （,t＇2=10.94,P〈0.05）, but no significant differences were found in the liver and spleen size and the portal vein diameter among these groups. Conclusion The short-term HAART has cardiovascular safety, nor could it aggravate splenomegaly of AIDS patients. Ultrasonography could accurately evaluate the function of left ventricle and abdominal organ＇ s morphological changes in AIDS patients after antiviral therapy.
