鼻内镜下鼻中隔成形术后应用纳吸棉和医用膨胀海绵的临床观察

摘　　要：目的比较观察鼻内镜下鼻中隔成形术后鼻腔填塞纳吸棉和膨胀海绵的临床效果。方法拟行鼻内镜下鼻中隔矫正术的鼻中隔偏曲患者160例,随机分为纳吸棉组和膨胀海绵组,术毕分别以纳吸棉或膨胀海绵填塞鼻腔,评估两组患者的术后不适感、填塞物取出后的相关症状、黏膜伤口愈合情况和远期并发症的差异。结果两组患者在局部疼痛、压力感、阻塞感、溢泪、喷嚏、发热等一般症状表现上无明显差异(P>0.05)。纳吸棉组患者在填塞物取出后鼻腔出血量和黏膜愈合情况明显好干膨胀海绵组(P<0.05),但远期并发症二组无明显差异。结论纳吸棉和膨胀海绵的填塞效果无明显差异,但纳吸棉可液化吸收而避免取出填塞物时的疼痛与出血,可以替代膨胀海绵而作为鼻腔填塞的新型材料。Objective To observe the clinical usability of biodegradable synthetic polyurethane foam （NasoPore） and Merocel for nasal packing during functional endoscopic septoplasty. Methods Included in this study were 160 cases undergone functional endoscopic septoplasty to receive nasal packing with different packing materials. They were randomly divided into two groups, with 80 patients in each group, and were packed with biodegradable synthetic polyurethane foam （NasoPore） for one group and with Merocel for the other group. Then, a comparative observation was made between these two groups to evaluate differences in uncomfortable sensation in local region, associated symptoms following the packing material withdrawn, injury healing situation and complications occurred during a longer post-operative period. Results There were no significant differences in the severity of general symptoms, such as local pain, pressure sensation, obstructive felling, lacrimation, sneezing and fever between these two groups （P〉0.05）. The quantity of nose bleeding and the speed of mucosa healing were both significantly superior for cases in NasoPore group to that in Merocel group once the packing material withdrawn postoperatively （P〈0.05）, but no significant differences were present in the long-term effect between these two groups as seen during the following up period lasted for 4 weeks postoperatively （P〉0.05）. Conclusion Biodegradable NasoPore holds the same clinical usability as that of Merocel. However, NasoPore can replace Merocel in clinical practice because it is a new type of biodegradable synthetic material so as to avoid painful feeling and bleeding when withdrawing nose packing material.
