无囊i-gel喉罩气道无肌肉松弛剂全麻在老年下肢手术中的应用  

Application of the Ⅰ-gel Laryngeal Mask Airway Without Muscle Relaxants on Elderly Patients With Lower Limb Surgery Under General Anesthesia

摘　　要：目的评价无肌肉松弛剂无囊i-gel喉罩全麻在老年下肢手术的应用效果。方法选择2009年7月—2011年5月我院择期行下肢手术67例老年患者,观察无肌肉松弛剂喉罩气道通气全身麻醉效果以及并发症。结果一次成功置入喉罩64例,其余3例中2例因漏气改用5号有囊喉罩置入成功,1例因患者咽喉反射强烈重新置入喉罩。所有患者均顺利完成手术。术后1例咽部疼痛,雾化处理24 h后缓解;无恶性心脑血管事件;无明确与喉罩相关的误吸事件。结论 i-gel喉罩操作简便,适合在老年无肌肉松弛剂喉罩全麻下肢手术中的应用。Objective To evaluate effect of the i-gel laryngeal mask airway without muscle relaxants on elderly patients with lower limb surgery under general anesthesia. Methods A total of 67 elderly patients, who would undergo lower limb surgery during July 2009 and May 2011 in our hospital, were observed effect and complication by laryngeal mask airway without muscle relaxants under general anesthesia. Results A total of 64 patients were inserted laryngeal masks successfully ; among the other 3 patients, a size 5 classic laryngeal mask was successfully used in 2 patients whose failure were due to large pharyngeal leak, and the another was reinserted laryngeal mask due to strong throat reflex. All patients underwent surgeries successfully. After operation 1 patient with sore throat relieved by atomization treatment 24 hours later; there were no severe cerebrovascular events and aspiration events related with laryngeal mask. Conclusion The i-gel laryngeal mask is feasible for airway without muscle relaxants on elderly patients with lower limb surgery under general anesthesia.
